摘　　要：目的：研究吸烟和遗传因素在西安地区非农业人口食管癌发生中的作用．方法：采用病例对照研究方法，于１９９４年８月到１９９５年１月，对８９例病例和９７例对照进行了调查．结果：食管癌患者中有吸烟和食管癌家族史的比例显著高于对照（Ｐ＝０．００００），单因素分析表明，吸烟（ＯＲ＝３．３）和食管癌家族史（ＯＲ＝１０．４）是引起西安地区非农业人口食管癌的主要危险因素．分层分析表明二者间存在交互作用，在食管癌的发生中似乎存在协同加强作用．结论：不同于食管癌高发区，吸烟和食管癌家族史是西安地区非农业人口食管癌发病的两个重要因素．提示有遗传背景的食管癌个体可能是在吸烟等环境因素的激发下发生的．Objective: The current study was initiated to determine the role of smoking and inherited factor in development of esophageal cancer. Methods: A case control study was undertaken from August, 1994 to January, 1995. All patients who have been histologically confirmed were sampled as cases in hospitals attatched to our university. Controls were selected from inpatients with non cancer diseases in Department of Orthopaedic and Physiotherapy in same hospitals. Results: Eighty nine cases and 97 controls were collected. Univariate analysis showed that smoking (OR=3.3) and family history of cancer (OR=10.4) were significantly related to esophageal cancer ( P =0.0000), and these two factors presented synergism. Conclusions: These findings show that both smoking and family history of cancer are important risk factors for esophageal cancer, if the influence of smoking was controlled, the effect of family history of cancer to esophageal cancer can reflect the role of inherited factor. The results strongly suggest that inherited susceptibility to esophageal cancer can represente under the condition of the specific environment (smoking), and the development of esophageal cancer of hosts who have inherited susceptibility may be induced by smoking habit.
