摘  要:目的:探讨胃癌患者血浆循环核酸p53基因突变检测的可行性,研究基因突变与胃癌的发生、发展、预后的关系。方法:采用聚合酶链反应(PCR)扩增p53基因外显子7,用变性高效液相色谱技术(DHPLC)对产物进行突变分析,再与各生物参数进行相关性研究。结果:在34例胃癌患者中11例(32.4%)发生p53基因突变,突变率与组织类型、组织分化程度有关(P<0.05),与临床分期、淋巴结转移及远处转移无关(P>0.05)。结论:检测胃癌患者血浆循环核酸p53基因突变,对胃癌的恶性程度、预后评估有一定的临床价值。而DHPLC可作为一种快速简便的基因检测手段。Objective:To evaluate the presence of tumor p53 gene mutation DNA in plasma from gastric cancer patients, establish a new approach to detect gene mutation in plasma DNA and study its clinical significance for the occurrence, development and prognosis of the cancer of stomach. Methods: DNA was extracted from plasma of 34 gastric cancer patients by using QIAGen Blood Kit and the target DNA fragments were amplified by PCR. The production were analyzed by denaturing high performance liquid chromatography (DHPLC); the mutation status obtained by DHPLC was proved by DNA sequenceing. Results : The target fragments was obtained from all samples of 34 gastric cancer patients. In 11 out of 34 patients mutation of the fragment were detected by DHPLC and confirmed by DNA sequencing. Conclusion:Plasma DNA can be amplified in gastric cancer patients . DHPLC is an accurate, rapid and economical way to screen mutation in plasma DNA amplified fragment.
