摘  要:目的探讨环氧化酶-2(COX-2)和缺氧诱导因子(HIF-1)在非小细胞肺癌(NSCLC)组织中的表达及其与肿瘤生物学特征和肿瘤血管生成的关系。方法免疫组化法检测NSCLC组织COX-2和HIF-1α的表达,并测定肿瘤微血管密度(MVD)。结果NSCLC组织中COX-2及HIF-1α阳性表达率分别为55.55%,71.11%,显著高于正常肺组织(P均〈0.05);有淋巴结转移组COX-2阳性表达率高于无淋巴结转移组;病理分级中Ⅱ-Ⅲ级组COX-2阳性表达率高于Ⅰ级组;肺腺癌组COX-2的阳性表达率高于肺鳞癌组,差异均有统计学意义(P均〈0.05);HIF-1α的表达与淋巴结转移及临床分期有关(P均〈0.05),与肿瘤的大小和肿瘤的分化程度无明显关系(P〉0.05);COX-2与HIF-1α、MVD,HIF-1α与MVD的表达之间均呈正相关(r=0.417,0.673,0.448,P均〈0.01)。结论COX-2和HIF-1α的增强表达与非小细胞肺癌的浸润性发展和肿瘤血管生成有密切关系,联合检测COX-2和HIF-1α可作为判断肺癌转移潜能的重要指标。Objective To investigate the relationship between the expression of COX-2, HIF-1 α and tumor biological characteristics, tumor angiogenesis in non-small cell lung cancer (NSCLC). Methods The expression of COX-2, HIF-1α and MVD were evaluated by immunohistochemical staining. Results The expression of COX-2 and HIF-1α in NSCLC tissue was demonstrated in 55.55% and 71.11% respectively, and significantly higher than that in normal lung tissue ( P 〈0. 05). The positive rate of COX-2 expression in the NSCLC tissues of patients with lymph node metastasis was significantly higher, compared with patients without lymph node metastasis. The positive rate of COX-2 expression in patients of pathological grade Ⅱ - Ⅲ was higher than those of grade I , and the positive rate of COX-2 expression in patients of adenocarcinoma carcinoma was higher than those of squamous cell carcinoma ( all P 〈 0. 05). The positive rate of HIF-1 α expression in patients with lymph node metastasis was significantly higher than those without metastasis, and the positive rate of HIF- 1 α expression in patients of clinical stage Ⅲ - Ⅳ was higher than those of stage Ⅰ - Ⅱ ( all P 〈 0. 05). There was a statistically significant correlation between the expression of COX-2, HIF-1 α and MVD. Conclusion The present finding suggests that high expression of COX-2 and HIF-1 α in NSCLC tissues may be related to tumor invasion and angiogenesis, the expression of COX-2 and HIF-1 α in NSCLC tissues may be an important marker for judgment of metastasis.
