16层CT重组技术对乳腺癌的诊断价值

摘  要:目的:研究16层CT重组技术对乳腺癌的诊断价值。方法:33例经病理证实乳腺癌患者均经16层CT扫描,然后进行1.25mm薄层重组,并进行MPR、MIP、3D及VR重组,并对33例16层CT扫描的薄层重组及后处理图像对乳腺癌的形态特征及其血管和淋巴结显示情况进行列表分析。结果:33例乳腺癌经16层CT扫描薄层重组与各种后处理的技术的应用显示形态不规则28例;分叶及毛刺30例;多灶性3例;密度不均匀14例。其中8例钙化,6例囊变,所有的病变均有明显强化,31例显示异常的肿瘤血管;肿块邻近皮肤、Cooper韧带增厚12例、乳头凹陷,导管增粗7例,9例肿块侵犯基底膜累及胸大肌;腋窝淋巴结增大18例,其中2例伴纵隔淋巴结增大。结论:16层CT的薄层重组及各种后处理方法比重组前图像提供更多图像信息,直观、形象、生动显示乳腺癌的特征,尤其是对肿瘤血管和沙砾状的钙化显示,弥补了常规CT扫描的缺陷,从而更有助于影像医生的诊断和临床医生的治疗。Objective:To study the diagnostic and reconstruction value of 16-slice CT in breast cancer patients. Methods:33 patients with pathologically confirmed breast cancer were studied. They were performed 16-slice CT and underwent thin slice and MPR. MIP.3D.VR reconstruction. Masses, vessels of carcinoma and lymph nodes were observed with 16-slice CT and underwent thin slice and MPR. MIP.3D.VR reconstruction. Results:In all the 33 cases of Breast cancer patients, 28 cases of irregular masses, 30 cases of burr and sublobe 30 cases, 14 cases were inconsistent with density in mass, there were 8 cases with calcification and 6 cases with cyst;3 cases with multi masses;unusual vessel of tumors was found in 31 cases; thickening of the skin and Cooper's ligament over the cancer was observed in 12 cases, and nipple retraction and duct dilatation occurred in 7 cases. Auxiliary lymph nodes were positive in 18 cases. In 2 cases out of the 18 were in mediastinum lymph nodes. Conclusion: 16-slice CT with their reconstruction is the best technique to observe the various manifestations of breast cancer and to detect the metastases in the surrounding lymph nodes.
