胃肠道间质瘤中E2F1和p16^(INK4α)蛋白的表达与预后的关系

摘　　要：目的探讨E2F1和p16INK4α在胃肠道间质瘤(gastrointestinal stromal tumors,GISTs)中的表达及两者的相关性,为GISTs的临床治疗和预后判断提供新的病理学依据。方法应用免疫组化EnVision法检测52例GISTs中p16INK4α和E2F1的表达情况,分析二者表达与Fletcher分级、进行性疾病(progressive disease,PD)及CD117和PKC-θ表达之间的关系,并对二者与GISTs预后的关系进行统计学分析。结果E2F1蛋白阳性率73.1%,与Fletcher分级呈显著正相关(P<0.001);E2F1阳性表达与CD117阳性表达之间有显著性差别(P<0.05);E2F1阳性细胞数大于75%的病例与小于75%的三组(<10%,10%-50%,50%-75%)之间PD发生率有显著性差异(P<0.05)。p16INK4α在52例GISTs中均有不同程度的阳性表达,与Fletcher分级呈显著负相关(P<0.001);p16INK4α阳性细胞数小于50%的病例与大于50%的两组(50%-75%,>75%)PD发生率之间差异有显著性(P<0.05)。结论E2F1、p16INK4α表达与GISTs预后有关,可作为GISTs预后的标记物。　Objective To investigate the expression of E2F1 and p16^INK4αin gastrointestinal stromal tumors（GISTs）,and to provide a new pathological evidence for evaluating clinical treatment and prognosis. Methods EnVision immunohistochemical method was used to detect the expression of E2F1 and p16^INK4αin 52 cases of GISTs,and to analyze their relationship with Fletcher grading,progressive disease（PD）,expression of CD117 and PKC-θ,and prognosis of GISTs.Results E2F1 expression was positively related to Fletcher grading of GISTs （P〈0.001）.The positive expression rate of E2F1 was 73.1%,and it was significantly different with CD117 positive expression（P〈0.05）.PD incidence in E2F1 expression〉75% was higher than in E2F1 expression〈75%（〈10%,10%-50%,50%-75%,P〈0.05）.p16^INK4αprotein expressed in different degree among 52 cases of GISTs,and the expression was negatively related to Fletcher grading of GISTs（P〈0.001）.PD incidence in p16^INK4αexpression〈50% was higher than in p16^INK4αexpression〉50%（50%-75%,〉75%,P〈0.05）. Conclusion The expression of E2F1 and p16^INK4αare closely related with the prognosis of GISTs,and may be considered as molecular markers for prognosis of GISTs.
