摘  要:目的分析前列腺增生、前列腺癌的MRI及三维MR波谱成像(3DMRSI)表现,探讨MRI联合3DMRSI对前列腺癌的临床应用价值。方法对17例前列腺增生和15例前列腺癌患者行MRI及3DMRSI成像,分析MRI表现及两组(Cho+Cr)/Cit峰值比值的差异。结果MRI清晰显示前列腺增生部位、大小及增生结节病理类型;并较好显示前列腺癌部位、周围组织侵犯情况和远处转移。前列腺增生和前列腺癌组MRS的(Cho+Cr)/Cit峰值比值分别为0.68±1.96、3.07±1.10,两者有显著性差异(p<0.001)。结论MRI、3DMRSI两者并用,将形态学和代谢产物的信息相结合,更有利于前列腺癌的诊断和定位。Objective To investigate the appearances and clinical value of MR imaging (MRI) added to 3D-MR spectroscopic imaging(3DMRSI) in prostate cancer. Methods MR imaging added to 3D-spectroscopic imaging was carried out in 17 benign prostate hyperplasia(BPH) and 13 prostate cancer(Pca). The apperances of MR imaging and the peaks of choline(Cho) citrate(Cit) creatine(Cr) were observed. The ratio of choline plus creatine to citrate { (Cho+Cr)/Cit } in prostate cancer were calculated and compared with those groups of BPH. Results MRI clearly demonstrated the position, size and pathological types of prostate hyperplasia. There was a significicant decrease in the the ratios of (Cho+Cr)/Cit in the Pca compared with the group of BPH(p〈0.05). Conclusion MRI added to 3DMRSI can provide the information of morphology and metabolite, which is more helpful to detecting and locating the lesions of Pca.
