摘  要:目的:探讨肌电图(EMG)对CT介入盘内置管多次多点注射臭氧治疗腰椎间盘突出症(PLID)效果的评价。方法:选取200例PLID患者,随机分为两组:臭氧组:120例,采用CT介入盘内置管多次多点注射臭氧治疗;对照组80例,采用传统的经皮腰椎间盘摘除术治疗。并用腓总神经、胫神经的神经传导速度(NCV)及远端潜伏期(DL)变化来客观地观察两组的治疗效果。结果:临床疗效:臭氧组120例中,优76例(63.3%),良27例(22.5%),中13例(10.8%),差4例(3.3%),总有效116例,有效率为96.7%;对照组80例中,优35例(43.8%),良23例(28.8%),中9例(11.3%),差13例(16.3%)。总有效67例,有效率为83.5%。两组患者治疗前双侧腓总神经、胫神经的NCV、DL延长。经治疗后,两组NCV均增快,DL缩短,但臭氧组变化更显著,且接近于正常值。与治疗前比较,经统计学处理,差异有非常显著意义(P<0.01);与对照组比较,差异有统计学意义(P<0.05),臭氧组120例患者腓总神经、胫神经NCV以及DL的改善明显优于对照组。结论:CT介入盘内置管多次多点注射臭氧可使PLID患者NCV增快,DL明显缩短,从而客观地说明患者神经肌肉功能改善。Objective: To explore the value of nerve conduction velocity(NCV) and distal latency (DL) monitoring pre-and post-treat protrusion of lumbar intervertebal by ozone injection. Methods: Two-hundred patients with prolapse of lumbar intervertebal dise(PLID) divided into two groups : the ozone injection therap group(ozone group) with 120 cases and the routine operation group(control group) with 80 cases. In the ozone group, the ozone 5-30ml injective into sick intervertebal disc under CT intervention; In the control group, routine operation enucleated sick lumbar intervertebal disc. EMG was examined at pre-and-post treatment between the two groups. Results: The clinical therapitic effect were found: in the ozone group 76 cases(63.3%)are excellent,27(22.5%) fine, 13(10.8%) moderate and 4(3.3%) inferior; the total effective rate being 96.7%. And in the control group 35 cases(43.8%) are excellent, 23(28.8%) fine, 9 (11.3%) moderate, and 4(16.3%) inferior, the total effective rate being 83.5%. After treatment, the NCVs of the common peroneal N. and the tibial N. increased, and the DLs were shortened. As compared with that before treatment, these result have obvious difference(P〈0. 01). NCV was faster in the ozone group patients than that sick dise in the control group . The nerve function improvement in the patients with sick dise ozone injection was better than that in the patients with routine to enucleated sick dise. Such improvement can be proved objectively by NCV and DL monitoring.
