胰头癌CT表现探讨（附16例分析）

摘　　要：目的：通过16例胰头癌CT表现分析提高胰头癌CT诊断鉴别诊断水平。方法：收集本院从2002年7月～2007年1月经CT诊断为胰头癌16例,其中手术证实6例,死亡4例,余6例正在治疗中。结果：主要CT表现：胰头局限性肿块,肿块边界不清,偶见胰腺弥漫性肿大,肿块中央可见低密度区,主胰管扩张,胰腺体尾部萎缩,胰腺周脂肪间隙消失,16例增强后病灶均呈不规则低密度改变。讨论：胰头癌为少血供肿瘤,其病理类型绝大多为腺癌。临床表现多样化,但以腹痛和黄疸为主。其诊断要点是：胰头部或钩突部局限性肿大,平扫大多数为等密度改变,少数中央出现低密度,主胰管扩张,大多数胰体尾部萎缩,胰头边界不清,胰周脂肪间隙消失。增强后肿块呈不规则低密度灶。若侵及胆总管致胆总管扩张及肝内胆管扩张。晚期浸及周围组织与腹膜后淋巴结肿大。本病应与慢性胰腺炎、壶腹癌、胆总管结石及转移癌鉴别。Objective： discriminate the diagnosis level through 16 pancreases head cancer CT performance analysis exaltation pancreas head cancer CT diagnosis. Methods： collections originally hospital from July of 2002 to was examine a patient by CT for the cancer 16 of the pancreas head in January 2007, among them, the surgical operation confirms 6, deads 4, remaining is 6 just in the under medical treatment. Results： main per- formance of CT：the pancreas head limits sex lump,the lump boundary is not pure, seeing the pancreas widespread swollen and big accidentally,the lump is central it is thus clear that low density area,the main pancreas tube extend,the pancreas body tail department atrophy,a fat cleft of pancreas disappear, 16 after examples strengthen the focus all presents the irregular and low density change. Conclusion： the pancreas head cancer provides the tumor for the little blood, its pathologic type cut off is the gland cancer mostly. Clinical performance diversification, but regard stomachache and jaundices as principle, It examine a patient important point is：The pancreas head or hook form departments limit sex swollen big, even sweep the majority in order to wait the density change, the minority is central to appear the low density, main pancreas the tube extend, most pancreases body tail department atrophy, the pancreas head boundary is not pure, a fat cleft of pancreas disappear, The lump presents the irregular and low density cooking stove after strengthen. If invade the total bile duct with the result that the total bile duct extend and the bile duct inside the liver extend. The later period immerse and the surroundings organize and peritoneum behind the lymphoid knot is swollen greatly, This disease should with chronic pancreatitis,a stone calculus with next stomach cancer,total bile duct and transfer the cancer to discriminate.
