肿瘤椎体转移的三维适形放疗与常规放疗初步比较

摘　　要：目的回顾性比较恶性肿瘤椎体转移三维适形放疗和常规放疗的近期疗效。方法将本肿瘤中心40例肿瘤椎体转移患者分为两组,分别采用三维适形放疗(3DCRT)和普通放疗治疗肿瘤椎体转移;三维适形放疗(3DCRT)采用42～45Gy/14～15次、5次/周的分割方式进行减症放疗,普通放疗采用30～36Gy/10～12次、5次/周的分割方式进行减症放疗。分别有12例15处病灶;28例37处病灶。结果全组病例有效率(CR+PR)为90.4%,两组的有效率依次为93.3%(14/15)、89.2%(33/37)。两组之间止痛效果差异无统计学意义。结论对恶性肿瘤骨转移进行局部减症放疗是一种有效的治疗手段,三维适形放疗与普通放疗的短期疗效差异无统计学意义。但三维适形放疗可改善靶区剂量分布在保护脊髓及周围正常组织方面具有重要意义,是局部剂量提升的有效方法。但对生存的影响有待更长时间的随访。Objective To evaluate the preliminary outcome of three-dimensional conformal radiation therapy（3DCRT） as compared with conventional radiation therapy（CRT） in spinal metastatic cancer. Methods 40 spiral metastatic cancer patients received radiotherapy in Daping Hospital Cancer Center,with 12 patients 15 cases in 3DCRT group and 28 patients 37 cases in CRT group. 3DCRT group were treated with 42- 45Gy/14 - 15fractions, 5 fractions/week; CRT group were treated with 30 - 36Gy/10 - 12fractions,5 fractions/week. Results The paliaitive therapy,with a total response rate of （CR＋PR）90.40/00. The response rate in 3DCRT,CRTgroups were 93.3 % （14/15）, 89.2 % （33/37） respectively. There was no significant difference between two groups. Conclusion Local radiotherapy is an effective paliaitive measure for spinal metastatic cancer. There is no significant difference in two groups. Three-DCRT improves the target volume coverage with signifcant sparing of the spinal cord and the other nearby critical normal structures. However,the long-term results need luther follow-up.
