摘 要:目的比较^(18)F-FDG PET/CT 与 CT 在非小细胞肺癌(NSCLC)原发灶(T)及淋巴结(N)分期中的价值。方法以术后病理诊断为标准,比较30例 NSCLC 患者^(18)F-FDG PET/CT 与 CT 在 T及 N 分期中的诊断效能。结果 ^(18)F-FDG PET/CT 和 CT 在 T 分期中准确性分别为86.7%,73.3%(P>0.05);对115站胸内淋巴结的诊断灵敏度、特异性及准确性分别为68.8%,95.2%,87.8%和43.8%,88.0%,75.7%,2种方法准确性差异有统计学意义(P<0.05);对65站纵隔淋巴结诊断灵敏度、特异性、准确性分别为90.5%,97.7%,95.4%和57.1%,81.8%,73.8%,2种方法准确性差异有统计学意义(P<0.01);在 N 分期中准确性分别为76.7%,66.7%(P>0.05)。结论 ^(18)F-FDG PET/CT 融合图像较 CT 更有助于 NSCLC 术前 T 和 N 分期,尤其在纵隔淋巴结的诊断中明显优于 CT。Objective It has long been recognized that clinical and pathological stage of non-small cell lung cancer (NSCLC)was related to the prognosis of the patients. The current study was designed to compare the diagnostic values of ^18F-FDG PET/CT versus CT alone in assessing the T and N staging of NSCLC. Methods Thirty cases of pathological proven NSCLC underwent beth ^18F-FDG PET/CT and chest CT before surgical operation. The diagnostic value of PET/CT versus CT in T and N staging was compared according to the findings in pest-surgical pathology. Results In T staging,the accuracy of is F-FDG PET/ CT and CT were 86.7% and 73.3%. In a total of 115 intra-thoracic nodal stations, the sensitivity, specificity and accuracy of PET/CT and CT were 68.8%, 95.2%, 87.8% and 43.8%, 88.0%, 75.7% respectively, the accuracy of the two methods had significant difference (P 〈 0.05). In 65 mediastinal nodal stations, the sensitivity, specificity and accuracy of PET/CT and CT were 90.5%, 97.7%, 95.4% and 57.1%, 81.8% , 73.8% respectively, the accuracy of the two methods had significant difference (P 〈 0. 01). Generally in N staging, the accuracy of PET/CT and CT was 76.7% and 66. 7% (P 〉0.05). Condusion Compared with CT, ^18 F-FDG PET/CT fused imaging was proved better in assessing the T and N staging of NSCLC, particularly in the diagnosis of mediastinal lymph nodes involvement.
