摘  要:目的总结甲状腺转录因子-1(TTF-1)在一组肺腺癌中的表达特点,分析其表达是否与肿瘤来源及分化有关,以及在肺腺癌的表达是否具有特异性。方法应用HE和免疫组化方法对133例肺腺癌的组织芯片行TTF-1、CDX2、ER、PR、PSA、CD10、Tg染色并观察,同时对照10例癌旁肺组织、14例肺转移性腺癌和5例其他部位肺转移性腺癌的石蜡包埋切片的染色结果。结果正常肺组织TTF-1表达定位于终末呼吸单位,而肺腺癌TTF-1阳性表达率为85.7%。低分化腺癌组TTF-1阳性表达程度低于高分化腺癌组(P=0.002);周围型腺癌组的阳性率高于中心型组(P=0.029)。结论TTF-1对于肺腺癌是一个有用的标记物,TTF-1表达可能与肿瘤发生于外周气道或肺泡上皮细胞有关。Objective To investigate the expression of thyroid transcription factor 1 (TTF-1) in different patterns of pulmonary adenocarcinoma and its relation to the origin of the tumour and to confirm the specificity of TTF-1 for pulmonary adenocarcinoma. Methods A tissue army including 133 cases of primary lung adenocarcinomas, 9 primary salivary adenocarcinomas, and 14 pulmonary metastatic adenocarcinomas, as well as 5 pulmonary adenocarcinoma metastasized to other organs was studied. Immtmohistochemical staining was performed on paraffin-embedded sections using anti-TTF-1 CDX2, ER, PR, PSA,CD10 and Tg antibodies. Results In normal lungs TTF-1 expressed in the terminal respiratory units. Furthermore, the expression was maintained in 85.7% of the adenocarcinoma. The staining in poorly differentiated adenocarcinomas was less than that in well - differentiated group ( P = 0.002). Of the peripheral adenocarcinomas exhibited higher positive expression than those of central bronchial origin ( P = 0.029). Conclusion TTF-1 staining is useful in the differential diagnosis of pulmonary adenocareinomas and it may be a lineage marker for tumours arising from the peripheral airway or alveolar epithelium.
