摘　　要：目的评价选择性动脉溶栓治疗急性脑梗死的疗效和安全性。方法2003年1月～2005年5月期间对25例急性脑梗死患者进行选择性动脉溶栓,同期选择年龄、病情相匹配的无溶栓的对照患者25例,同时采用卒中登记方法收集选择性动脉溶栓患者及同期无溶栓的对照患者的临床、实验室及影像学等资料,前瞻性随访所有患者6个月的预后。结果溶栓组和对照组1个月末的病死率分别为4%和4%,6个月末的病死率分别为4%和4%;溶栓组和对照组1个月末残疾率分别为16%和40%,6个月末的残疾率分别为4%和24%。溶栓组并发脑出血2例(8%),其中症状性脑出血1例(4%);对照组未发生症状性脑出血。结论选择性动脉溶栓治疗急性脑梗死患者有效和安全。Objective To evaluate the effect and safety of selective intra-arterial thrombolysis （SIAT） for acute ischemic stroke（AIS）. Methods Selected AIS patients for SIAT. Through stroke registry, Clinical experimental and imaging information of SIAT and non-thrombolytic control patient were collected at the same time,and all patients were followed-up to evaluate the prognosis after 6 months prospectively. Results SIAT were carried out on 25 AIS patients during January 2003 through May 2005,25 non-thrombolytic control patients matched on age and severity were selected during this period. The mortality of the SIAT and controlled patients were 4% and 4% respectively at the end of 1 month,and the mortality of the SIAT and control patients were 4% and 4% respectively at the end of 6 month. The disability of SIAT and control patients were 16% and 40% respectively at the end of 1 month,and the disability of SIAT and control patients were 4% and 24% respectively at the end of 6 month. Intracerebral hemorrhages were happened to 2（8%） cases among SIAT patients, 1（4%） of them was symptomatic;None symptomatic intracerebral hemorrhage was happened to control patients. Conclusions Compared to control patients, SIAT was effective and relatively safe for AIS patients.
