抗癫痫药物治疗癫痫对血浆性激素水平变化的影响

摘　　要：【目的】了解癫痫患者的血浆性激素水平变化情况;服抗癫痫药物对于癫痫患者血浆性激素水平的影响。【方法】用电化学发光法检测入组癫痫患者的泌乳素、雌二醇、孕酮以及总睾酮的血浆水平,并分别比较男、女患者未用抗癫痫药物组与用药组的血浆性激素水平的变化。【结果】入组92例患者,男性41例,女性51例;51.09%(47/92)有PRL水平升高,43.48%(40/92)患者E2水平升高,分别有18.48%(17/92)和25.00%(23/92)患者的TTes、PG水平下降。女性癫痫患者中,AEDs组患者有77.14%(27/35)出现E2升高,NAEDs组为43.75%(7/16),两组有显著差异(P<0.05);男性癫痫患者中,AEDs组31.58%(6/19)出现TTes水平下降,NAEDs组为4.55%(1/22),两组有显著差异。【结论】癫痫患者的性激素水平有变化;抗癫痫药物可影响性激素水平。男女患者的性激素受抗癫痫药物的影响变化不同,男性患者的TTes水平降低,女性患者的E2水平升高可能与抗癫痫药物的应用有关。[Objective]To investigate changes of plasma sexual hormone concentration in patients with epilepsy and evaluate the effect of anti-epileptic drugs on plasma sexual hormone levels in epileptics. [Methods] All the participants were drawn blood sample for the analysis of plasma prolactin （PRL）, estradiol （E2）, progesterone （PG）and total testosterone （TTes）, and then plasma sexual hormone levels in male and female patients with treatment were respectively compared with those without treatment. [Results]The study was performed in 92 epileptic patients （male 41, female 51）. The levels of PRL and E2 increased in 51.09%（47/92） and 43.48% （40/92） patients, respectively . The concentrations of PG and TTes decreased in 25.00 % （23/92） and 18.48%（17/92） patients, respectively. Plasma E2 concentration was high in 77.14%（27/35） female patients with anti-epileptic drugs, and in 43.75%（7/16） female patients without treatment. There were significant differences between two groups（ P 〈0.05）. TTes was low in 31.58% （6/19） male patients with anti-seizure treatment, and in 4.55% （1/22） male patients without treatment. The difference is significant between two groups. [Conclusion]There are changes of plasma sexual hormone concentration in patients with epilepsy . Anti-epileptic drugs affect plasma sexual hormone concentration. The effects on sexual hormone are different in male and female patients with anti-epileptic drugs. The level of TTes decreases in male patients and the level of E2 elevates in female patients, which may be related to use of anti-epileptic drug.
