大剂量顺铂三联在口腔颌面部鳞癌化疗中的应用

摘　　要：目的比较经颞浅动脉插管灌注小、中、大剂量顺铂联合平阳霉素、长春新碱静脉给药在术前化疗口腔颌面部鳞癌中的疗效。方法将59例口腔颌面部鳞癌随机分为3组:A组7例,B组15例,C组37例,手术前分别经颞浅动脉插管灌注小、中、大剂量顺铂加静脉给予平阳霉素、长春新碱联合化疗,共2疗程,观察其疗效及毒副反应。结果术前化疗口腔颌面部鳞癌后,A、B、C三组缓解率分别为42.8%、66.7%和91.9%。C组与A、B两组相比,差异有统计学意义(P<0.05)。结论经颞浅动脉插管灌注大剂量顺铂联用平阳霉素、长春新碱对口腔颌面部鳞癌的术前化疗疗效优于中、小剂量组,使肿瘤易于手术切除,副反应轻,适合临床推广使用。Objective To compare the effect of small, medium, large dose of cis-platinum infusing through arteria temporalis superficialis cannula plus Pingyangmycin and Vinblastine intravenous administration in the preoperative chemotherapy of dermoid cancer in oromaxillo facial region. Methods 59 patients with dermoid cancer in oromaxillo facial region were randomly divided into three groups （Groups A, B and C）, each group was treated with cisplatinum in different dosages infusing through arteria temporalis superficialis cannula combined with intravenous administration of Pingyangmycin and Vinhlastine. The duration was two weeks. The sizes of tumor were measured and the blood count and hepatic and renal function were detected. Electrocardiogram was performed before and after chemotherapy. Results After chemotherapy, the remission rates were 42.8% and 66.7%, and 91.9% in Groups A, B and C, respectively. There was statistical significance between Group C and other groups. The side effect of chemotherapy was endurable. Conclusion The effect of large dose of cis-platinum infusing through arteria temporalis superficialis cannula combined with Pingyangmycin and Vinblastine intravenous administration on treating dermoid cancer in oromaxillo-facial region before operation was better than that of small and medium dosage. After chemotherapy, the exairesis becomes easier with less side effect. This scheme of chemotherapy can he applicable in clinic.
