减重步行训练对脑梗死偏瘫患者康复疗效的影响

摘　　要：目的探讨减重步行训练对脑梗死后偏瘫患者下肢运动功能、步行功能、ADL能力及生活质量的影响。方法将132例脑梗死偏瘫患者随机分为减重步行训练组(63例)及对照组(69例),2组患者均接受常规康复训练,减重步行训练组在此基础上同时进行减重步行训练。2组患者于康复治疗前后分别采用功能性步行量表(FAC)、Fugl-Meyer运动功能量表(FMA)、Barthel指数评分(BI)评定患者步行能力、下肢运动功能及ADL水平;治疗结束后随访3个月,每月均采用生活质量量表(SF-36)对患者生活质量进行评定。结果治疗后2组患者下肢运动功能、步行能力及ADL水平均较治疗前明显改善,并且减重步行训练组上述各项指标改善幅度均明显优于对照组(P<0.05);随访第2,3个月时,发现减重步行训练组SF-36评分均明显高于对照组(P<0.05)。结论减重步行训练不仅有利于脑梗死偏瘫患者步行功能及ADL能力恢复,而且还有利于其生活质量的提高;在脑梗死偏瘫患者康复治疗过程中,应结合患者实际病情尽早对其实施减重步行训练。Objective To investigate the effect of partial body-weight supported treadmill training （PBWSTT） on function of lower limbs, walk function, ADL performance and quality of life of hemiplegic patient induced by cerebral infarction. Methods A total of 132 cerebral infarction patients were divided into a control group （ n = 69） and a training group（n =63） randomly. Both groups accepted routine rehabilitation therapy, and the training group accepted PBWSTT at the same time in addition. Both groups were evaluated with regard to their walking ability, function of lower limbs, ADL performance and their quality of life by using Functional Ambulation Category （FAC） , Fugl-Meyer assessment （ FMA）, Barthel index （BI） and SF-36 before and after rehabilitation treatment. Results The function of lower limb, walking ability, ADL performance and the quality of life of both groups were improved significantly after treatment, and those in the training group were improved to a significantly greater extent than those in the control group （P 〈0.05 ）. Conclusion PBWSTT was beneficial not only to the improvement of walking and ADL performance, but also to improvement of quality of life in stroke patients.
