摘 要:目的:研究VEGF-C及其VEGFR3在乳腺癌组织及癌周组织中的表达情况、分布定位及其在乳腺癌淋巴结转移中的作用。方法:采用免疫组化SP法检测70例患者乳腺癌组织及其癌周组织中VEGF-C和VEGFR3的表达情况,CK19染色检测腋窝淋巴结微转移情况。结果:乳腺癌70例,VEGF-C在乳腺癌组织阳性表达率为78.6%,在癌周组织中的表达率为54.3%(P<0.01)。VEGFR3阳性的脉管数在乳腺癌组织为7.90±5.57,瘤周组织中为10.31±6.52(P<0.01)。VEGFR3的表达主要受乳腺癌细胞VEGF-C表达的影响,两者具有明显的相关性(r=0.66,P<0.05)。乳腺癌淋巴管主要定位于癌周组织及肿瘤间质内,癌巢内未发现成熟淋巴管。VEGF-C在乳腺癌组织内的阳性表达率淋巴结微转移阳性组为87.5%,在淋巴转移阴性组为71.1%,两组差异有统计学意义(P<0.01)。VEGFR3阳性脉管数在乳腺癌瘤周组织中淋巴转移阳性组为12.48±6.51,淋巴转移阴性组为8.47±6.02,两组差异有统计学意义(P<0.01)。结论:VEGF-C在乳腺癌组织肿瘤细胞中呈高表达,而其受体VEGFR3则表达乳腺癌间质和癌周组织的淋巴内皮细胞;乳腺癌新生淋巴管主要定位于癌周及间质;VEGF-C在乳腺癌组织、VEGFR3在癌周组织的表达与乳腺癌淋巴浸润和淋巴结转移密切相关。Objective:To study the expression and location of VEGF-C and its receptor VEGFR3 in breast carcinoma tissues, as well as their roles in lymph metastasis. Methods: SP method was used to examine the expression of VEGF-C and VEGFR3 in 70 cases of breast cancer and in its peritumoral tissue, and tested the expression of CKI9 in lymph node to explore lymph metastasis. Resuits -In all 70 cases of breast cancer,the positive expression rate of VEGF-C in breast carcinoma tissue was 78.6% ,and its rate in peritumoral tissue was 54.3 % ( P 〈 0.01 ). The positive lymph vessel of VEGFR3 was higher in peritumoral tissue ( 10. 31±6. 16 ) than that in breast carcinoma tissue(7.90 ±5.57 ) (P 〈 0. 01 ). The expression of VEGFR3 in peritumoral tissue was closely correlated with VEGF-C expressed in breast carcinoma tissue( r = 0. 66, P 〈 0. 05). The positive lymph vessel of VEGFR3 was located in tumor stroma tissue and peritumoral tissue, and mature lymphatic vessels were not found in cancer-nest. The positive expression rate of VEGF-C in lymph node metastasis group was 87.5%, and in no lymph node metastasis group was 71.1%. There was a significant difference between the two groups ( P 〈 0. 01 ). In peritumoral tissue, the VEGFR3 positive vessels in lymph node metastasis group ( 12.49 ±6. 51 ) was significantly higher than in non-lymph node metastasis group (8.47 ±6. 02) (P 〈 0. 01 ). Conclusion: The positive expression of VEGF-C is located in breast cancer cells, and of VEGFR3 is located in lymphatic vascular endothelial ceils of tumor stroma tissue and peritumoral tissue. The newly lymphatic vessels of breast cancer were located in tumor stroma tissue and peritumoral tissue. VEGF-C and VEGFR3 are closely correlated to the lymphangiogenesis and lymph node metastasis of breast cancer.
