晚期直肠癌Miles术后会阴部复发的综合治疗（附8例报告）

摘　　要：为探讨低位直肠癌术后会阴部复发的综合治疗方法，回顾分析8例直肠癌Miles术后会阴部复发患者的临床资料。经B超、CT及MRI证实会阴部为孤立性病灶，行局部姑息性超范围切除，配合化疗、中药内服加外洗、局部换药、心理治疗等进行综合治疗。结果显示，患者会阴部疼痛、酸胀、分泌物等症状明显缓解，异物感消失，生活质量明显提高。结果表明，直肠癌术后会阴部复发，由于病变局限，手术操作容易，行姑息性超范围切除，并配合其它辅助治疗能够收到良好的治疗效果，提高了术后生存率，应重视直肠癌术后会阴部复发的治疗。The comprehensive treatment of the recurrence at the perineal area after Miles operation of advanced rectal carcinoma in 8 patients was summarized retrospectively. The perineal recurrence of advanced rectal carcinoma was confirmed by B-ultrasonography, CT and MRI as isolated lesions which were ultra-extensively resected locally. Then chemotherapy, decoction of traditional Chinese materia medica （for intake and washing） ,local change of dressing as well as psycological treatment were followed. As results, the symptoms of perineal pain, swelling and fluid secretion were notably relieved, the foreign body sensation disappeared,and patients life quality was improved. It is suggested that palliative and ultra-extensive resection of the recurred lesion of rectal carcinoma at perineal area is a simple approach in dealing with such abnormalites,and that comprehensive therapy is able to enhance survival rate with better life quality.
