臀部肌注致坐骨神经损伤的神经电生理诊断价值研究

摘  要:目的:探讨神经电生理检测在臀部肌注致坐骨神经损伤诊断中的应用价值。方法:对89例患者进行腓总神经、胫神经的运动传导速度检测,并对胫前肌、腓肠肌、股二头肌长短头行针极肌电图检测。结果:89例病例,腓总神经MCV异常58例(65.16%),胫神经异常21例(23.59%),腓总神经、胫神经同时异常10例(11.24%)。肌电图示:患者失神经电位改变胫前肌有68例(76.4%),股二头肌短头有65例(73%);腓肠肌有34例(38.2%),股二头肌长头失神经电位改变有32例(35.96%)。经统计学处理,与正常对照组比较有显著意义(P〈0.01)。结论:神经电生理检测对臀部肌注致坐骨神经损伤的诊断可提供客观依据。Objiect: To investigate the diagnostic values of neuro - electrophysiological tests on sciatic nerve injury following gluteal intramuscular injection. Methods : 89 patients were examined of motor nerve conduction velocity on common peroneal nerve and tibial nerve, and electromyograms were tested on their musculus tibialis anterior, musculus gastrocnemius and musculus biceps femoris. Results: There were 58 cases of abnormal MCV on common peroneal nerve, 21 cases on tibial nerve and 10 cases on both common peroneal nerve and tibial nerve. 68 patients occurred neurolosting potiential changes on musculus tibialis anterior, 65 patients occurred on the shorter end of musculus biceps femoris, 32 patients occurred on the longer end of musculus biceps femoris, and 34 patients occurred on musculus gastrocnemius. Statistical significances were found comparing patients to normal controls (P 〈 0.01 ). Conclusion: Neuro - electrophysiological tests may afford objective evidences on diagnosing sciatic nerve injury following gluteal intramuscular injection.
