自酸蚀粘接剂复合树脂修复后牙Ⅰ类洞三年的临床评价

摘　　要：目的评价自酸蚀粘接剂复合树脂修复后牙Ⅰ类洞的临床效果。方法70例中深龋磨牙分为两组,实验组36例采用自酸蚀粘接剂+混合填料复合树脂修复,对照组34例使用全酸蚀牙本质粘接剂+可充压复合树脂修复。结果在复查的三年期限内,两组修复体在术后敏感性、修复体表面形态、边缘密合性及继发龋方面均无显著性差异,色彩匹配性在术后一年和二年的统计学结果有显著性差异(P<0.05)。结论用自酸蚀粘接剂和复合树脂修复后牙Ⅰ类洞可获得同样满意的临床效果,但混合填料复合树脂组色彩稳定性较可充压复合树脂组略差。Objective To evaluate the clinical effects of self-eching primer and composite resin on posterior ClassⅠrestorations.Methods Seventy molars with medium to deep caries from 23 patients （6 males, 17 females） were divided into two groups. The experimental group： 36 cases were restored with Clearifil SE bond （self etching bond） ＋ Clearfil AP-X （hybrid composite）; The control group： 34 cases were restored with Prime ＆ Bond NT（total etching dentin adhesive） ＋ Esthet-X（packable composite）. The restorations were assessed after placement at 1 week, 6 months, 1 year, 2 years and 3 years to check post-operative sensitivity, color match, anatomic form, marginal adaptation and secondary caries according to Rege＇s criteria.Results Post-operative sensitivity of molars was not present in the experimental group, only 1 case had temporary sensitivity after 1 week in the control group. There was no significant difference between two groups on sensitivity （P〉0.05）. Color match of restorations in both groups had Bravo grade appeared at different stages, no case was scored as Charlie. Eighteen restorations were scored as Bravo in the experimental group and 6 in the control group during 3 years. The results of color match of 1 year and 2 years between the experimental group and the control group had statistically significant differentes （P〈0.05）. There was no statistically significant difference between the two groups at anatomic form, marginal adaptation and secondary caries （P〉0.05）, all of them were considered well.Conclusion This study showed that using two different types of adhesive and different composite resin on posterior classⅠ restorations could get satisfied clinical results, the color stability of packable composite resin was better than hybrid composite resin.
