消化道憩室98例的临床病理分类

摘　　要：目的探讨消化道憩室的病因病理分类．方法消化道憩室９８例全部为手术切除标本，常规病理取材，切片，ＨＥ染色．对憩室不同横断面和发生憩室的消化道壁作仔细观察．结果以憩室部位分类：食管憩室２例，胃憩室６例，十二指肠憩室１２例，空肠憩室１１例，回肠憩室５５例，盲肠憩室５例，降结肠、乙状结肠憩室各２例，阑尾憩室３例．单发性憩室９０例，多发性憩室８例．病因学分类：先天性憩室５１例，内压疝出性憩室３８例，内压扩张性憩室５例，外牵性憩室３例，炎症修复性憩室１例．病理组织学分类：真性憩室５３例，假性憩室４３例；２例内压扩张性憩室的室壁有残留平滑肌纤维，称过渡性憩室．１１例憩室内合并异位消化腺，１例憩室外回肠有异位消化腺．结论理顺了消化道憩室的分类方法．确立了过渡性憩室的存在．AIM To classify the diverticulum of alimentary tract aetiologically and histopathologically. METHODS Ninety eight samples were taken during operation and pathological sections were stained by HE. The various cross secitons of the diverticulum and the walls of alimentary tract where the diverticulum occurred were carefully observed. RESULTS Esophageal diverticula were found in 2 cases, gastric diverticula 6, duodenal diverticula 12, jejunal diverticula 11, ileal diverticula 55, cecal diverticula 5, the diverticula of descending colon and sigmoid 2 each, and appendiceal diverticula 3. Single diverticulum was found in 90 cases, and diverticulosis in 8 cases. Aetiologically they were classified into 5 groups: congenital diverticula, 51 cases; herniated diverticula because of inner pressure, 38 cases; dilated diverticula because of inner pressure, 5 cases; tractional diverticula, 3 cases; and inflammatory reparative diverticulum, 1 case. Histopathologically, they were classified into 3 groups: true diverticula 53 cases; pseudodiverticula, 43 cases; and transitional diverticula, 2 cases. The vestiges of smooth muscle were observed in the wall of transitional diverticula which belong to the dilated diverticulum because of inner pressure. Aberrant digestive glands were found on diverticula in 11 cases and on ileum outside the diverticulum in 1 case. CONCLUSION The classification method for diverticulum of alimentary tract should be systemized and the transitional diverticulum be recognized.
