盖诺加顺铂适形放疗治疗Ⅲa期非小细胞肺癌疗效观察

摘　　要：目的观察盖诺加顺铂联合三维适形放射治疗(3DCRT)局部晚期非小细胞肺癌的耐受性及近期疗效。方法36例局部晚期非小细胞肺癌(Ⅲa期)患者进入化疗加三维适形放射治疗组,全部患者均行盖诺加顺铂方案(盖诺25mg/m2第1,8天给予;顺铂25mg/m2第1～3天给予;21d为1个周期)化疗加3DCRT同步治疗。结果36例均完成治疗患者,肺原发灶CR占16.7%,PR占75.0%,NR+PD占8.3%,总有效率为91.7%;纵隔转移淋巴结CR占36.4%,PR占63.6%,NR+PD占0%,总有效率为100%;白细胞下降发生率为95.8%,其中3,4级白细胞下降为36.1%;放射性食管炎和放射性肺炎的发生率分别为54.2%和12.5%,均为1,2级。结论盖诺加顺铂联合适行放疗治疗局部晚期非小细胞肺癌能为绝大多数患者耐受,有较好的近期疗效。Objective To evaluate the lately efficacy and tolerability of three-dimensional conformal radiation therapy （3DCRT） combined with NP regimen chemotherapy for limited advanced stage non-small cell lung cancer（NSCLC）. Methods Thirty-six patients with limited advanced stage non-small cell lung cancer were enrolled for this study, NP regimen chemotherapy （vinorelbine were administered 25 mg/m^2 on day 1 and 8;cisplatin 25 mg/m^2 on day 1 to 3 ,which was recycled every 3 weeks） combined with 3DCRT were performed in all cases. Results For pulmonary primary tumor,the complete remission（CR） rate was 16.7 %, the partial remission （PR） rate was 75.0%, the no change and progression disease （ NC ＋ PD） rate was 8.3 %, the over- all response（CR ＋ PR） rate was 91.7%. For mediastinal lymph nodes metastasis, the CR rate was 36.4%. PR rate was 63.6%, the （NC ＋PD） rate was 0% ,the （CR ＋PR） rate was 100%. Leucopenia accounted for 95.8% ,in which grade HI and IV accounted for 36.1%. The radiation esophagitis and radiation pneumonia accounted for 54.2% and 12.5% respectively, all was grade I or ]]. Conclusions NP regimen chemotherapy combined with 3D-CRT has well lately efficacy for NSCLC and can be tolerated by most patients.
