男性精神分裂症患者情绪识别障碍与执行功能障碍的关系

摘　　要：目的探讨男性精神分裂症患者情绪识别障碍与执行功能（EF）障碍之间的关系。方法选择41例精神分裂症男性住院患者以及年龄、受教育年限与之相匹配的33例正常对照,采用中国面部表情视频系统（CFEVS）测试情绪识别功能,威斯康星卡片分类测验（WCST）、Stroop字色干扰测验（SCWT）和言语流畅性测验（VFT）评定执行功能。结果患者组情绪识别正确数[（71.7±17.1）个]及反应时间[（2.25±0.75）s]均比对照组差,对照组分别为（80.8±16.2）分、（1.25±0.61）分（P〈0.05）,且各项EF测验结果均比对照组差（P〈0.01）,且均与病程无关（P〉0.05）。2.识别中性情绪（咀嚼、无表情面孔）正确数与WCST完成第一分类所需应答数、Stroop彩色文字阅读（C）和彩色文字颜色阅读CW时间呈负相关（P〈0.05）;识别喜悦、悲伤面孔正确数和总正确数与C和CW时间呈负相关（P〈0.05）,与Stroop（C）和CW正确数以及VFT总分呈正相关（P〈0.05）;识别总反应时间与VFT总分呈负相关（P〈0.05）。结论男性精神分裂症患者存在明确的情绪识别障碍,以识别中性情绪困难为主,其严重程度与执行功能障碍呈平行关系,两者在病程中呈稳定状态;SCWT及VFT测验可作为精神分裂症情绪识别障碍的生态学标记。Objective To explore the relationship between emotion identification and executive functions in male patients with schizophrenia, Method 41 male schizophrenic inpatients and 33 normal controls matched in age and education level were assessed with Chinese Facial Expression Video System （CFEVS） and a battery of neuropsyehologieal tests （WCST＆SCWT＆VFF） measuring executive functions. Results The male patients with schizophrenia performed worse than normal controls on all tasks of facial emotion recognition and execution functions which were both not correlated with the course（ P〈 0.05）. Recognition accuracy showed significant negative correlations with the scores of Pf and SCWT-time ,and showed positive correlations with SCWT-aeeuraey and the total scores of VFT which were also positively correlated with the response time of recognition（ P 〈0.05 ）. Conclusion The male patients with schizophrenia were significantly impaired in emotion perception, especially in neutral emotional faces, which was parallelly correlated with executive functions. Both were in a stable state during the phase of disease. SCWT and VFT could be taken as ecologic marks of the disorders of emotion perception in schizophrenia.
