急性过敏性缝线反应临床观察

摘　　要：目的观察眼肌手术急性过敏性缝线反应发生概率，探讨其发生原因。方法观察斜视患者138例，采用常规手术方法，依据术前检查制定具体手术方式，术中后徙术选用5—0丝线缝合，缩短术选用3～0丝线缝合。结果急性过敏性缝线反应发生率8．696％，多发生在儿童和青年女性斜视缩短术后。结论临床统计急性过敏性缝线反应和文献记栽相比发生率偏低，而且急性过敏性缝线反应多发生在截除手术后，占全部发生患者总数的83．33％，原因在于缩短术比后徙术所用的缝线较多，且更靠近角膜缘和葡萄膜，而急性过敏性缝线反应发生的原因可能和缝线的开始分裂和被吸收有关，属于不典型的Ⅳ型变态反应。Objective In order to study the probability of the acute anaphylaxis of sutures, and discussion the reason, we observed the acute anaphylaxis of sutures in extra - ocular muscle operation. Methods We studied 138 strabismal patients, used the normal operative methods, and designed the operative method. In the operation, we use 5 - 0 silk to saturation in recession techniques, 3 - 0 silk in shortening procedure. Results The rate of the acute anaphylaxis of sutures is 8. 696%. It more appeared on children and youth female in the shortening procedure. Conclusions To compare the rate of clinical result and reference, the rate of clinical result is down. The acute anaphylaxis of sutures more appeared in shortening procedure. The constituent ratio is 83.33%. The reason is that the using sutures is more great and more near limbus and uvea in the shortening procedure than in the recession techniques. The cause of the acute anaphylaxis of sutures maybe is that sutures was decomposed and absorbed. The acute anaphylaxis of sutures is un - typical delayed hypersensitivity.
