乳腺囊内乳头状癌临床病理学特征的探讨

摘　　要：目的探讨乳腺囊内乳头状癌(intracystic papllary carcinoma,IPC)的临床病理学特征。方法采用常规苏木精-伊红(HE)染色、光镜观察,并通过Maxvision法对16例乳腺IPC进行免疫组化检查。结果乳腺IPC的细胞为高柱状,与乳头轴心垂直排列。细胞排列紧密,核位于基底,卵圆形,核苍白或深染。在一些病例中,肌上皮细胞消失。还有些病例,尤其是肿瘤较大的病例中,纤维脉管轴心有广泛的玻璃样变性及纤维化。免疫组化显示乳头被覆细胞CK7、p53、Ki-67、E-cadherin均阳性,CD34、actin和SMA乳头纤维血管轴心及周围血管呈阳性表达,而actin、ER、PR、cerbB-2、p63及CK,(34βE12)癌细胞均阴性。结论乳腺IPC好发于老年妇女,即绝经期后的妇女,具有较独特的病理组织学改变和免疫表型,乳腺IPC的预后非常好。Objective To explore the clinicopathological characteristics of intracystic papllary carcinoma of the breast. Methods Tissues of intracystic papllary carcinoma from 16 patients studied pathologically, The specimens were treated by routin HE staining,and then studied microscopically, Immunohistochemical study by Maxvision method was also performed. Results The cells of intracystic papllary carcinoma of the breast were in high columnar form, vertical aligned with the papillary stalk. The nucleus which was oval and deeply or palely stained ,were located in the basilar part, The myoepithelial cells were disappeared in some cases; the tumor cells lost polarity, and normal stratified epithelium was replaced by the proliferative cells. Wide-range hyalinosis and fibrosis in the fibrovascular core were also seen in some cases, especially in the cases with large tumors. Immunohistochemical staining demonstrated that tumor cells overlapped by papillae were positive for CK7 ,p53 ,Ki-67 and E-cadherin,and fibrovascular cores of papillae and peripheral vessels were positive for CD34,actin and SMA. No expression of actin, ER,PR, cerbB-2 ,p63 and CK were noted in all tumor cells. Conclusion Intracystic papllary carcinoma of the breast most occurs in postmenopausal women with distinct histo-pathologic changes and immunophenotypes. The patients with intracystic papllary carcinoma of the breast often have a good prognosis.
