凋亡抑制基因存活素在膀胱癌组织中的表达及意义

摘　　要：目的探讨凋亡抑制基因存活素(Survivin)在膀胱癌组织中的表达及其意义。方法应用免疫组化(SP)法,对60例膀胱移行细胞癌组织标本中存活素的表达进行检测,并分析其表达与膀胱移行细胞癌临床病理特征的关系。结果60例膀胱癌组织中存活素阳性表达率为65.00%,10例正常膀胱组织存活素表达均为阴性。Ⅰ、Ⅱ、Ⅲ级膀胱癌组织中存活素阳性表达率分别为37.50%、68.96%、86.66%,Ⅰ级与Ⅱ、Ⅲ级比较均有显著性差异(P<0.05),T1、T2、T3期膀胱癌组织中存活素阳性率分别为54.54%、70.00%、85.71%,呈逐渐升高趋势,但组间比较均无显著性差异(P>0.05)。复发性膀胱癌存活素阳性率为75.00%。结论存活素与膀胱癌发生发展有关,检测膀胱癌组织中存活素表达,对判断膀胱癌预后有一定意义,可以作为判断膀胱癌发生发展、有无复发的指标之一。Objective To study the expression of apoptosis inhibitor gene Survivinin the bladder carcinoma tissues and its clinical implications. Methods We detected the expression of Survivinin the tissues from 60 patients with bladder transitional cell carcinoma and analyzed the relationship between it＇ s expression and the clinicopathological characteristics. Results The positive rate of Survivinin the bladder carcinoma tissues from 60 patients was 65.00%. No Survivinwas expressed in the normal bladder tissues from 10 healthy controls. The positive rates of Survivinin grade Ⅰ , Ⅱ and Ⅲ bladder carcinoma were 37.50% , 68.96% and 86.66% , respectively. The positive rate of Survivinin grade Ⅰ was significantly lower than that in grade Ⅱ and　Ⅲ （P 〈 0.05 ）. The positive rate of Survivinin T1, T2 and T3 was 54.54% ,70.00% and 85.71% , respectively, which showed a trend of increased expression of Survivinwith more advanced stages, but there was no statistically significant difference among them （ P 〉 0.05 ）. The positive rate was 75.00% in the tissues from patients with recurrent disease. Conclusion Survivinmay play an essential role in the carcinogenesis and development of bladder carcinoma. Detection of the expression of Survivinin the tissues from bladder carcinoma may be helpful in evaluating the prognosis of the patients. It may also be used as one of the markers in monitoring the relapse.
