摘  要:目的:观察腹腔注射曲马朵对脊神经部分结扎大鼠神经病理性疼痛的影响及其可能的作用机制。方法:雄性SD大鼠64只,随机分为8组(n=8),分别为假手术组、等渗盐水对照组、曲马朵5 mg/kg组、曲马朵10 mg/kg组、曲马朵20 mg/kg组、纳络酮预处理组、育亨宾预处理组和纳络酮+育亨宾联合预处理组。采用热辐射法和vonFrey细丝法分别测定大鼠热刺激缩足反射潜伏期(TWL)和机械刺激缩足反射痛阈值(MWT)。结果:与术前和假手术组相比,对照组大鼠在术后第4 d开始TWL变化值明显增大,MWT明显降低,并持续至术后第7 d(P<0.05或P<0.01);与对照组相比,曲马朵10 mg/kg、20 mg/kg组的TWL和MWT均显著延长或增大(P<0.05或P<0.01);与曲马朵10 mg/kg、20 mg/kg组相比,纳络酮和育亨宾预处理组的TWL和MWT均显著缩短或减小(P<0.05或P<0.01);与曲马朵10 mg/kg、20 mg/kg组、纳络酮和育亨宾预处理组相比,纳络酮+育亨宾预处理组的TWL和MWT均显著缩短或减小(P<0.05或P<0.01)。结论:腹腔注射曲马朵10 mg/kg、20 mg/kg对脊神经部分结扎大鼠机械性痛觉过敏和热痛觉过敏具有剂量依赖性治疗作用,其抗伤害感受性作用可被纳络酮或育亨宾部分拮抗;曲马朵治疗神经病理性疼痛的机制可能不仅与阿片受体、α2肾上腺素受体相关,应还有其他机制的参与。Objective: To observe the effect of Tramadol ip injection on neuropathic pain induced by ligation of the Ls spinal nerve (SNL) in rats. Methods : Sixty-four male SD rats weighing 250 - 320 g were randomly divided into 8 groups of equal number: a sham operation group, a control group treated with SNL, saline ip. , 3 Tramadol treated groups (ip, 5 , 10 and 20 mg/kg, respectively), a Naloxone pre-treated group, a Yohimbine pre-treated group and a Naloxone + Yohimbine pre-treated group. Thermal withdrawal latency (TWL) and mechanical withdrawal threshold (MWT) of the rats were determined by Hagreave' s thermal radiation apparatus and von Frey' s method of microfilament, respectively. Results:TWL and MWT decreased in the control group from the 4th to the 7th post-operation day (P 〈 0.05) as compared with the sham operation group, extended and augmented respectively in the Tramado110 and 20 mg/kg groups ( P 〈 0.05 and P 〈 0.01 ) as compared with the control group, shortened and diminished respectively in the Naloxone and Yohimbine pre-treated groups ( P 〈 0.05 and P 〈 0.01 ) as compared with the Tramadol 10 and 20 mg/kg groups, and decreased in the Naloxone + Yohimbine pretreated group (P 〈0.05 and P 〈0.01 ) as compared with the Tramadol 10 and 20 mg/kg groups and the Naloxone pre-treated and Yohimbine pre-treated groups, all with statistic significance. Conclusion: Tramadol can alleviate the mechanical and thermal hyperalgesia induced by SNL in rats in a dose-dependent manner, and the mechanism of its anti-ociceptive action may be partially associated with the opiate receptor and α-adrenoreceptor.
