摘 要:目的探讨肥胖青少年胰岛素抵抗和B细胞分泌功能的变化。方法对9969名11~19岁的中学生行肥胖筛查,纳入肥胖267例,超重185例,正常136例,分别行葡萄糖耐量试验,同时测量腰围、臀围,计算腰臀比、腰高比;据空腹和服糖30min、120min血糖、胰岛素值计算稳态模型胰岛素抵抗指数(HOMA-IR)和胰岛素分泌功能(HOMA-β、△Ins30/△Glu30)。结果3组腰围、臀围、腰臀比、腰高比和血胰岛素值均有显著性差异(P〈0.0001),餐后30min血糖在肥胖和正常组间有统计学差异(P〈0.0248);3组间HOMA-IR、HOMA-β、△Ins30/△Glu30比较均有显著性差异。结论肥胖和超重者存在高胰岛素血症,可能存在胰岛素抵抗,肥胖青少年p细胞早相分泌功能可能受损。Objective It is to explore insulin resistance and β cell secretion in obese adolescents. Methods A standard glucose-tolerance test was administered to 267 obese ones, 185 overweight ones, and 136 normal ones chosen from 9969 middle school students of 11 - 19 years. Baseline measurements included the weight, height, waistline, Hip-circumference and BMI, WHR〈 WHTR. Serum glucose and insulin levels were determined at 0, 30, 120 min after OGTT, insulin resistance and secretion of β cell were estimated by homeostasis model assessment of insulin resistance (HOMA- IR) and secretion of β cell (HOMA-β, △Ins30/△Glu30). Results There were significant differences among the three groups in waistline, Hip-circumference, WHR, WHTR and serum insulin levels (P 〈 0. 000 1). Compared with that in obese group, the difference in serum glucose at 30 min after OGTT in normal group was significant (P 〈 0.05), There were significant differences in HOMA- IR, HOMA - 13, △Ins30/△Glu30 among the three groups (P 〈 0, 01), Conclusion The obese and overweight groups all have hyperinsulinemia, so there may be insulin resistance and impairment of early secretion of β - cell in obese adolescents.
