后房型人工晶状体植入术矫治中高度近视临床应用

摘　　要：目的观察有晶状体眼后房型可植入接触人工晶状体矫治中、高度近视的有效性和安全性。方法中、高度近视患者21例38眼,术前屈光度-4.0^-22.6D,其中2眼-4.0D。术前充分扩大瞳孔,倍诺喜眼液表面麻醉,9点透明角膜缘用一次性3mm角膜切开刀做隧道切口,前房注入黏弹剂,植入折叠式人工晶状体,置于透明晶状体与虹膜之间,吸出黏弹剂后用卡米可林注射液缩瞳,在11点或1点处虹膜根部做部分切除。结果38眼术后24h均达到术前矫正度数的视力。结论可植入后房型人工晶状体是在保留透明晶状体的基础上植入的人工晶状体,角膜损伤小,保留生理性调节,具有可逆性、预测性好、术后视力恢复快、适用屈光范围广等优点,这种新方法的应用开辟了屈光手术治疗的新前景。　Objective To study the efficacy and safety of implantable collamer lens（ICL）implantation for the correction of moderate to high myopia.Methods Thirty-eight eyes of twenty-one patients with moderate to high myopia（from -4.0 to-22.6 diopters）had a ICL implanted.Prior to surgery,the pupils were enlarged adequately and topical anesthesia was administered with Benoxil.A tunnel incision was created at nine o’clock of cornea with 3 mm lamellar keratoplasty,and healon was injected into anterior chamber.Folded intraocular lens was implanted between transparent lens and iris.And then helon was removed,iridotomy was administered at eleven or one o’clock on the base of iris.Results All the patients reached corrected visual acuity in 24 hours postoperatively.Conclusion ICL implantation is administered on the basis of preserving the natural lens.It is predictable,safe,and effective in the correction of high myopia,and its indications should be carefully selected.
