γ-刀联合淋巴因子激活杀伤细胞治疗脉络膜黑色素瘤初步观察

摘　　要：目的：观察γ－刀联合淋巴因子激活杀伤细胞（ｌｙｍｐｈｏｋｉｎｅ－ａｃｔｉｖａｔｅｄｋｉｌｉｎｇｃｅｌｓ，ＬＡＫ细胞）治疗脉络膜黑色素瘤（ｃｈｏｒｏｉｄｍａｌｉｇｎａｎｔｍｅｌａｎｏｍａ，ＣＭＭ）的效果。方法：对５例ＣＭＭ患者在ＭＲＩ定位下进行多个放射中心联合照射，另外用ＬＡＫ细胞球后注射。通过眼底、影像学及淋巴细胞亚群等检查，进行６～２４个月随访。结果：瘤体变性、萎缩、完全消退者４例，其中治疗２年后者２例，２０、１４个月者各１例。瘤体缩小３／５左右，瘤区内的血流不显示的部分消退者１例，为治疗６个月以内者。所有病例未见明显并发症。结论：本疗法治疗ＣＭＭ具有疗程较短、副反应轻、疗效较为满意等优点，是保留眼球治疗ＣＭＭ的有效方法。OBJECTIVE:To observe the therapeutic efficacy of γ knife/lymphokine activated killing cells(LAK)in choroid malignant melanoma(CMM).METHODS:Five cases of CMM had been treated by retrobulbar injection of LAK cells and γ knife irradiation at multiple sites.Ophthalmologic,imageologic,fundus fluorescein angiographic and T lymphocyte subset examinations were done before and after treatment.The follow up period of this series of cases was 6-24 months. RESULTS: The CMM of 4 in 5 treated cases became atrophic and withered up clinically after γ kinfe/LAK therapy.Among the 4 cases,2 of them had been followed up for more than 2 years,and the other 2 for 20 and 14 months respectively.The tumor of the 5th patient who was followed up for 6 months after treatment,reduced to 3/5 of the original size,and no blood flow was found within the tumor mass under the clinical examination.CONCLUSION:The γ knife/LAK therapy was effective in treating CMM in saving the affected eye from being enucleated.
