摘  要:目的:研究VEGF和MMP-2在骨巨细胞瘤(GCT)组织中的表达情况及其与GCT血管生成和增殖状态的关系,并分析各种因素在GCT预后中的作用.方法:采用SP法检测85例患者标本VEGF,MMP-2,PCNA,CD34表达,分析其与GCT转移、复发的关系,并采用Cox回归模型分析GCT复发的影响因素.结果:85例GCT患者中VEGF,MMP-2的阳性表达率分别为62.4%和56.5%,增殖指数(PI)值为(52.0±21.5)%,微血管密度(MVD)值为(33.4±9.6)条.不同VEGF表达组间PI均值,MVD均值差异有统计学意义(P<0.01).不同MMP-2表达组间MVD均值差异有统计学意义(P<0.01).不同VEGF表达组,不同MMP-2表达组比较,其转移率、复发率差异有统计学意义(P<0.01).MVD值在有无转移组、有无复发组之间差异有统计学意义(P<0.01).PI值有无复发组之间差异有统计学意义(P<0.05).多因素Cox回归分析显示,VEGF(RR=6.056),MMP-2(RR=5.387),MVD(RR=3.121)是影响GCT复发的危险因素;与常规刮除术相比,广泛完整切除术(RR=0.176)是影响GCT复发的保护因素.结论:VEGF,MMP-2表达与骨巨细胞瘤的血管生成、细胞增殖及复发有关,可作为判定骨巨细胞瘤预后的参考指标.AIM: To investigate the expression patterns of vascular endothelial growth factor (VEGF) and matrix metalloproteinase 2(MMP-2) in giant cell tumors of bone( GCT), and their relationship with tumor proliferations and angiogenesis, then to analyze the roles of various factors in the prognosis of GCT. METHODS: The expressions of VEGF, MMP-2, PCNA, CD34 in 85 cases were examined with immunohistochemical staining and their relationship with metastases and recurrence of GCT was analyzed, meanwhile the influencing factors of recurrence were studied with Cox regression model. RESULTS: The positive rates of VEGF and MMP-2 in 85 GCT cases were 62.4% and 56.5%, respectively. PI value and microvessel density(MVD) value were (52.0 ± 21.5 ) % and (33.4 ± 9.6 ), respectively. PI value and MVD value between VEGF positive group and negative group were significantly different ( P 〈 0.01 ). MVD value between MMP-2 positive group and negative group was significantly different (P 〈 0.01 ). The metastases rate and recurrence rate between VEGF positive group and negative group, MMP-2 positive group and negative group were significantly different ( P 〈 0. 01 ). MVD value between metastases group and non-metastases group, recurrence group and non-recurrence group were significantly different (P 〈 0. 01 ). PI value between recurrence group and non-recurrence group was significantly different (P 〈 0. 01 ). Multiple factorial Cox regression analysis indicated that VEGF (RR = 6.056), MMP-2(RR =5. 387), MVD(RR =3. 121 ) were risk factors of recurrence of GCT; compared with routine apoxesis, general and complete ectomy ( RR = 0.176) was a protective factor of recurrence of GCT. CONCLUSION: The expressions of VEGF and MMP-2 are highly related to the recurrence of GCT, and could be used to predict the recurrence of GCT and to guide the clinical therapy of GCT.
