摘　　要：目的比较准分子激光原位角膜磨镶术(LASIK)与准分子激光上皮瓣下角膜磨镶术(LASEK)的临床疗效。方法LASIK患者506例(506只眼),LASEK患者539例(539只眼)入选统计分析。将两种手术病例都分成2组,组Ⅰ①、E①为低中度数组(等效球径值≤-6.00D),组Ⅰ②、E②为高度数组(等效球径值>-6.00D)。观察患者两种手术方式术后效果,并加以对比。结果术后疼痛LASIK组明显少于LASEK;LASIK术后10天视力≥0.8明显多于LASEK;术后半年达到最佳矫正视力、屈光回退LASIK与LASEK的差异无统计学意义;术后眩光在低中度数组差异无统计学意义,在高度数组LASIK明显多于LASEK。结论LASIK术后疼痛症状轻、视力恢复迅速,但高度数患者术后眩光的几率大;LASEK术能有效减少高度数患者术后眩光的出现,但是术后疼痛症状较重、恢复较慢。Objective To compare laser in situ keratomileusis （LASIK） and laser epithelial keratomileusis （LASEK） on clinical. Methods 506 eyes of 506 cases of LASIK, 539 eyes of 539 cases of LASEK were involved in the study. Laser machines used in the study is lasersightLSX. The LASIK hinge position was routinely at noseside, The application time of alcohol for LASEK was 20-25 seconds. Both LASIK and LASEK were divided into two groups, I （1）group and E（1） group were the groups of isoeffect spherical ≤-6.00D, I （2）group and E（2）group were the groups of isoeffect spherical〉-6.00D, Observe and compare the postoperative pain, recovery of effective vision, achievement to best-corrected visual acuity （BCVA） after six months of operation, refractive outcomes, glare etc of LAS1K and LASEK. Results Postoperative pain： LASIK group is less than LASEK group, P〈0.05, effective vision≥0.8 after 10 days of operation： LASIK group is more than LASEK group, P〈0.05, There was no significant difference in achievement to best-corrected visual acuity （BCVA） after six months of operation, refractive outcomes, In low myopia there was no significant difference in glare, P〉0.05, But in hige myopia glare was more usually happened in LASIK than in LASEK, P〈0.05. Condusions The condition of postoperative pain is less, recovery of effective vision is quicker and more stable after LASIK, But it requires high quality technique in making the hinge, and the condition of glare is more usually happened in high myopia group in LASIK. LASEK is more safe in operating progress. But the condition of postoperative pain is more and recovery of effective vision is slower after LASEK. There was no significant difference in refractive outcomes and the achievement of BCVA after six months of operation.
