口腔扁平苔藓上皮细胞凋亡和细胞增殖的表达及关系的研究

摘　　要：目的通过研究口腔扁平苔藓患者上皮细胞凋亡、增殖状况及P53,CyclinD1蛋白的表达,探讨OLP发病的机理。方法采用甲基绿派诺宁法检测30例OLP患者及20例正常对照者口腔上皮细胞凋亡的阳性表达,用免疫组化SABC法检测P53,CyclinD1,ki67蛋白在两组上皮中的分布和表达。结果①正常组与OLP组凋亡细胞阳性指数分别为0.69±0.25,1.98±1.07,t=5.28(p=0.03<0.05),有统计学意义;②正常组与OLP组P53蛋白阳性指数分别为5.15±3.12,20.73±10.56,t=6.35(p=0.001<0.05),有统计学意义;③正常组与OLP组中CyclinD1阳性指数分别为8.89±1.81,16.89±8.00,t=5.68,p=0.001<0.05有统计学意义;④正常组与OLP组中ki67蛋白阳性指数分别为14.02±6.75,31.22±12.19,t=5.7374,p=0.001<0.05,有统计学意义;⑤P53,ki67蛋白阳性细胞指数与凋亡指数呈正相关。其相关系数分别为r=0.8209,P<0.01,r=0.8349,P<0.05有统计学意义。结论①细胞凋亡及凋亡相关因子P53蛋白的异常表达可能导致OLP的病变。②细胞增殖与细胞凋亡贯穿了OLP发病的全过程,二者的平衡失调导致了OLP病变的反复发作。Objective To investigate the pathogenesis in oral lichen planus （OLP）, we study the expression of apeptosis, cell proliferation and p^53, CyclinD1 protein in oral epithelium mucosa in OLP patients. Methods 30 oral hchen planus specimens and 20 normal oral mucosa were immunohistochenmically stained with CychnD1, ki67, p^53 protein to count the positive expression. Results （1）The positive apoptosis apeptosis index of normal and OLP specimens are 0.69±0.25, 19.8±1.07, t=5.28,（p=0.03〈0.05）, they have obvious statistics difference;（2）he positive pss expression ratio of normal and OLP specimens are 5.15±3.12, 20.73±10.56, t=6.35 （P=0.001〈0.05）, they have obvious statistics difference; （3）The positive CyclinD1 index of normal and OLP specimens are 8.89± 1.81, 16.89±8.00, t=5.68, p=0.001〈0.05, they have obvious statistics difference; （4）The positive ki67 index of normal and OLP specimens are 14.02±635, 31.22±12.191, t=5.7374 p=0.001〈0.05, they have obvious statistics difference; （5） P^53 and apeptosis, ki67 and apeptosis are positive correlation. The relative quotiety are： r=0.8209, p〈0.01, r=0.8349, P〈0.05. Conclusions （1）Apeptosis and apeptosis related genes may participate in clinical development of OLP; （2）Apeptosis and cell proliferation are connected with all the pathological changes in OLP, whose imbalance contrinute to the formation and development in OLP.
