摘 要:目的探讨鼻咽癌(NPC)多体素氢质子磁共振波谱(1H-MRS)表现及其临床价值。资料与方法对10名健康志愿者和22例NPC患者行二维多体素1H-MRS检查,采用点解析波谱(PRESS)序列。原发肿瘤、转移淋巴结及正常组织的MRS用Sun ADW4.0工作站Functool软件处理分析。结果22处原发病灶中,18处获得可析性波谱,胆碱/肌酸(Cho/Cr)的平均值为2.81±1.32;14处转移性淋巴结中,12处获得可析性波谱,Cho/Cr的平均值为3.06±1.73。原发肿瘤与转移性淋巴结的Cho/Cr比值明显高于正常肌肉(P<0.001)。结论多体素1H-MRS能无创性观察NPC病灶的代谢改变,Cho/Cr比值可以作为NPC诊断的参考指标之一。Objective To discuss the multivoxel proton MR spectroscopy manifestations of nasopharyngeal carcinoma (NPC) and to evaluate its clinic values. Materials and Methods 10 healthy volunteers and 22 patients of NPC were examined with a 1.5T MR imaging unit. Multivoxel proton MR spectroscopy was performed with PRESS sequence in nasopharyngeal primary tumor, metastatic lymph node and normal tissue. Metabolic ratios of Cho/Cr were postprocessed using functool software. Spectra from normal neck muscle of ten volunteers were acquired as control data. Results Successful spectra were acquired in 18 (81%) of 22 primary tumors and 12 (86%) of 14 metastatic lymph nodes. Cho/Cr ratios were significantly higher in the primary NPC lesions ( P 〈 0.001 ) and metastatic lymph nodes ( P 〈 0.001 ) than that in muscle tissue ( primary tumor Cho/Cr = 2. 81 ± 1.32 , lymph node Cho/Cr = 3.06 ± 1.73 , muscle Cho/Cr = 0.82 ±0.19). Conclusion ^1HMRS is a useful tool for monitoring the tumor metabolic changes of NPC noninvasively, Cho/Cr ratio can be used as reference in diagnosis of NPC.
