乏脂肪肾血管平滑肌脂肪瘤与肾细胞癌的CT鉴别诊断

摘　　要：目的探讨乏脂肪肾血管平滑肌脂肪瘤(RAML)与肾细胞癌(RCC)的CT鉴别诊断征象,提高乏脂肪RAML的诊断水平。资料与方法回顾性分析经手术病理证实并行CT平扫及双期增强扫描的乏脂肪RAML28例、RCC34例患者的资料。对各项CT观察指标进行统计学分析。结果单发或多发、病灶与皮质交角、假包膜及"黑星征"等4项CT观察指标在乏脂肪RAML与RCC组中的差异有统计学意义。结论病灶的单发与多发、与皮质的交角、假包膜及"黑星征"对两者的鉴别诊断有重要价值。Objective To improve the diagnostic accuracy of renal angiomyolipoma （RAML） absent of fat by evaluating the CT manifestations of renal angiomyolipoma absent of fat and renal cell carcinoma （RCC）. Materials and Methods The CT manifestations of 28 cases of RAML absent of fat and 34 cases of RCC confirmed pathologically were retrospectively analyzed. All cases were performed with plain CT scan and dual - phase contrast - enhanced CT scan. Then statistical analysis was made. Results Four indexes were found to be significant and evident for differentiating RAML absent of fat from RCC, which were ＂single or multiple＂, angle with cortex, pseudocapsule, ＂black star＂ sign. Conclusion The ＂single or multiple＂, angle with cortex, pseudocapsule and ＂black star＂ sign are valued in the differentiation of RAML absent of fat and RCC.
