摘 要:目的:检测前列腺素过氧化物合成酶2(cyclooxygenase-2,COX-2)mRNA在慢性根尖周炎病损组织中的表达及其与病理学表现之间的关系,并对COX-2的表达进行细胞定位,探讨COX-2在慢性根尖周炎发病中的作用。方法:采用RT-PCR法,从mRNA水平检测COX-2在10例慢性根尖肉芽肿及3例根尖囊肿组织中的表达,3例因阻生拔除的健康第3磨牙牙周膜组织作为对照,分析COX-2的表达与慢性根尖周病损的病理学表现(包括病损的病理分型、炎症细胞浸润程度)之间的关系,并分析COX-2与病损范围、临床症状之间的关系。采用免疫组化的方法,对COX-2在病损组织中的表达进行细胞定位。结果:在根尖肉芽肿病损和根尖囊肿组织中,COX-2mRNA与β-actin水平的比值较正常牙周膜组织明显增高,分别为0.36±0.29和0.51±0.07,但根尖肉芽肿和根尖囊肿组织之间差异无统计学意义。COX-2mRNA在中度炎症细胞浸润组较轻、重度炎症细胞浸润组明显增高(0.76±0.11vs0.11±0.01,0.76±0.11vs0.23±0.06,P<0.05)。有临床症状的根尖周病损组的COX-2mRNA有增高的趋势,COX-2mRNA与病损大小无关。免疫组化结果显示,COX-2主要在浸润的炎症细胞及上皮细胞中表达,血管内皮细胞未见表达。结论:COX-2在慢性根尖周炎的发病中具有重要的作用,但也提示存在其它细胞因子的协同作用。Objective:To quantify COX-2 mRNA in lesions of chronical apical periodontitis and relate the findings to the histological findings.Methods:A total of 13 periapical lesions including 10 periapical granulomas and 3 periapical cysts were obtained.Normal periodontal ligament tissues from extracted third molors were used as control tissues.The expression of COX-2 mRNA was detected by reverse transcription polymerase chain reaction(RT-PCR)technique.The relationships between the expression of COX-2 mRNA and histological findings(including the type of histology,the degree of inflammatory infiltrates),the size of radiolucent of the lesions and the clinical fingdings of the involved tissues were analyzed.The expression of COX-2 at cell level was detected by immunohistochemistry.Results:A significant higher level of COX-2 mRNA was found in periapical granulomas and in periapical cysts,but no significant difference was detected between those two groups.COX-2 mRNA in the group with moderate inflammation cell infiltration was higher than that of light and heavy inflammation cell infiltration(0.76±0.11 vs 0.11±0.01,0.76±0.11 vs 0.23±0.06,P〈0.05).The elevated COX-2 mRNA levels were detected in lesions with clinical symptoms.COX-2-positive cells were detected in the epithelial cells and inflammatory cells.Conclusion:COX-2 might play more important roles in the pathogenesis and development of periapical lesions.
