早期肠内营养对食管癌患者术后康复的影响

摘　　要：目的观察早期肠内营养（EEN）对食管癌患者术后恢复的影响。方法随机将186例食管癌术后的患者分为两组，其中试验组92例施行EEN，对照组94例未施行EEN。分别对两组患者的术后并发症的发生率、血清白蛋白、前清蛋白、转铁蛋白，术后肠功能恢复时间，以及血清丙氨酸氨基转移酶、胆红素、肌酐和血糖进行统计分析。结果186例患者术后出现并发症28例，其中试验组8例（8．7％），对照组20例（21．3％），两组比较差异有统计学意义（P〈0．01）。试验组血清清蛋白、前清蛋白、转铁蛋白在术后第5、9天与对照组比较差异有统计学意义（P〈0．05）。试验组术后排气和排便时间明显早于对照组。两组术后肝肾功能、血糖差异均无统计学意义。结论EEN能明显降低食管癌患者术后并发症的发生率，显著改善术患者后的营养不良状况，促进患者的康复。Objective To observe the effects of early enteral nutrition （EEN）on postoperative recovery in patients with esophageal carcinoma. Methods One hundred and eighty-six patients with esophageal carcinoma were randomly divided into two groups. Patients in experimental group were given EEN （ n = 92）, control group were given non-early enteral nutrition （ N-EEN, n = 94 ）. The postoperative complications were recorded. The serum albumin, prealbumin, transferrin, alanine aminotransferase （ALT）, bilirubin, creatinine and glucose in both groups were measured on postoperative day 1,5 and 9, respectively. Resuits The total complications occurred in 28 patients,8 in experimental group,20 in control group. In experimental group, the serum albumin, prealbumin and transferrin were significantly higher than those in control group on postoperative day 5 and day 9, respectively （P 〈 0.05 ）. The time of exhaust and defecation in experimental group were earlier than those in control group. The serum ALT, bilirubin, creatinine and glucose had no significant difference between two groups,respectively. Conclusion EEN can significantly decrease the incidence of postoperative complications, improve nutritional status and accelerate the recovery of postoperative patients with esophageal carcinoma.
