摘 要:目的通过分析慢性过敏性皮肤病患者血清中七种食物过敏原特异性抗体IgG水平,探讨食物过敏原在慢性过敏性皮肤病诊断与预防中的意义。方法采用酶联免疫吸附试验(ELISA)检测285例本院确诊的慢性过敏性皮肤病患者和30例正常人血清中七种食物特异性IgG抗体水平。结果285例过敏性皮肤病患者一种或一种以上食物特异性IgG阳性患者226例,阳性率79.3%,30例健康对照组中2例阳性,阳性率6.7%,2组相比差异有统计学意义(P〈0.001)。七种食物特异性IgG阳性率从高到底依次是蛋清/蛋黄42.5%,花生36.5%,虾32.3%,牛奶24.93%,大豆20.7%,大米13.7%,小麦10.5%。对226例阳性患者中120例患者追踪随访,60%患者停相关食物1~2个月后症状好转。结论慢性过敏性皮肤病患者血清中存在食物特异性IgG抗体,此种抗体的存在对慢性过敏性皮肤病的临床确诊与预防食物过敏有重要的指导意义。Objective In order to investigate the significance of food allergens in diagnosis and prevention of disease, 7 kinds of serum specific IgG against food allergens were measured in patients with chronic allergic skin disease. Methods ELISA method was applied to determine 7 kinds of serum specific IgG against food allergens in 285 patients with chronic allergic skin diseases as well as 30 normal subjects. Results 226 of 285 cases was found food specific IgG positive, the positive rate was 79. 3%, while only two of 30 normal cases were positive and the positive rate was 6. 7%. There was significant difference between the positive rate in patient group and normal group ( P 〈 0. 001 ). The rates of seven kinds of food IgG were the followings: egg 42.5%, peanut 36. 5%, shrimp 32. 3% , milk 24.9%, soja 20. 7%, rice 13.7%, wheat 10. 5%. 120 of 226 positive cases (60%) had symptom relief after 1 -2 months since eliminating those food in their diet in the following visiting. Conclusion The detection of food specific IgG has a significance in diagnosis and prevention of chronic allergic skin disease.
