硫喷妥钠联合机械通气救治癫痫持续状态的临床探讨

摘　　要：【目的】探讨癫痫持续状态的治疗方法,评价硫喷妥钠联合机械通气抢救癫痫持续状态的疗效。【方法】45例患者依据临床特征、脑电图确诊为癫痫持续状态,入院后立即行硫喷妥钠1.0 g加入0.9%氯化钠溶液中,使用微量泵3-8 mg/min静滴。同时采用气管插管机械通气,根据血气分析合理选择呼吸模式和调节呼吸机参数,加强呼吸道管理。【结果】45例于5-30 min内抽搐全部控制,3-24 h内意识转清,生命体征、血气分析均恢复正常。【结论】使用硫喷妥钠联合机械通气控制癫痫持续状态快速、高效、安全。[Objective]To evaluate the effect of thiopental combined with artificial ventilation on status epilepticus, and explore an effective therapy for status epilepticus. [Methods] Forty five cases of status epilepticus were finally comfirmed by clinical examination and eletroencepalograph （EEG）. Thiopental （5%） was intravenously injected via microinfusion pump proceeded by trachea intubation and artificial ventilation immediately after admission. The optimal ventilation patterns and parameters were set up guided by blood gas analysis. [Results] The seizure of all 45 cases were relieved successfully within 5-30min after treatment. All patients recovered consciousness within 24 hours with normal vital signs and blood gas. [Conclusion] Thiopental combined with artificial ventilation is fast, effective and safe in the treatment of status epilepticus.
