三维适形放射治疗胰腺癌临床疗效观察

摘　　要：目的应用三维适形放射治疗技术治疗胰腺癌，评价其疗效及毒副反应。方法24例胰腺癌全部采用三维适形放射治疗，常规分割，总剂量60～70Gy，42～49d。结果治疗结束2～3个月后评价近期疗效，CR7例，占29．2％；PR14例，占58．3％；NC3例，占12．5％。总有效率（CR＋PR）达87．5％。中位生存期13个月，1年生存率58．3％。毒副反应主要为恶心、呕吐及白细胞减少，均可耐受。结论三维适形放射治疗胰腺癌近期效果满意，毒副反应轻，为失去手术机会的胰腺癌患者提供了一种有效治疗手段。Objective To evaluate the efficacy and toxicity of three dimensional conformal radiotherapy （3-DCRT） for pancreatic cancer. Methods Twenty-four patients with pancreatic cancer received 3-DCRT with conventional fraction, 2.0 Gy per fraction, five fractions per week. The total dose was 60-70 Gy for 42-49 days. Results The complete response （CR） rate was 29.2% （7/24） , the partial response （PR） rate was 58.3% （14/24） and the no change（NC） rate was 12.5% （ 3/24 ） in two or three months after 3-DCRT. The overall response （ CR ＋ PR） rate was 87.5%. The median survival duration and 1-year survival rate were 13 months and 58.3% respectively. The treatment was tolerated well. The side effects were observed as nausea, vomiting and leuxoeytopenia. Conclusions 3-DCRT is a safe and effective treatment of patients with advanced pancreatic cancer. The better local control rate and long-term survival rate can be expected.
