摘  要:目的:探讨对伽玛刀治疗功能性垂体腺瘤的疗效及相关因素。方法:回顾性分析伽玛刀治疗的412例功能性垂体腺瘤患者的临床资料。其中207例为PRL型,89例GH型,46例为ACTH,70例为混合型;肿瘤所接受的平均边缘剂量分别为30.6Gy、22.1Gy、34.5Gy、和21.7Gy。结果:平均随访时间为24个月,90例(40.2%)内分泌恢复正常,119例(53.1%)下降,无变化或升高15例(6.7%),84例肿瘤消失,117例(52.2%)肿瘤缩小,1例(0.4%)增大。结论:伽玛刀治疗功能性垂体腺瘤是安全有效的方法,不仅能控制肿瘤生长,而且能使内分泌功能大部分恢复正常。Objective: To evaluate the effect and the factors associated with gamma knife treatment for functional pituitary adenomas. Methods: Between June 1999 and June 2005,412 cases of functional pituitary adenoma were treated by gamma knife. 224 cases were followed up. Among them, 207 there were patients with prolactin (PRL)-producing tumors. 89 patients with growth homone (GH)- producing tumors. 46 patients with adrenocorticotrophic (ACTH)-producing tumors. 70 patients with tumors that secreted both PRL and GH. The mean marginal dose was 30. 6Gy, 22.1Gy, 34.5Gy and 21. 7Gy respectively. Results: The mean following-up time was 24 months. In 90 cases (40.2%) there was normalization of hormone secretion after gamma knife treatment. The hormonal level was reducted in 119 (53. 1%) patients. In 15 patients(6. 7%) there was no significant change. In radiological follow up 84 cases showed no tumor, in 117(52. 2%) cases the tumor reduced in size, in 1 case(0. 4%) the tumor increased in 1 case. Conclusion: Gamma knife radiosurgery is an effective and safe therapy both in tumor volume control and in hormone control.
