摘 要:目的探讨高温高湿环境下守岛官兵训练前后脑血流和脑电生理的变化,为临床防治热致疾病提供依据。方法从守岛官兵中随机抽取在常温常湿环境下[温度(27.40±1.37)℃,湿度(50.25±6.76)%]新兵100名为常温新兵组,高温高湿环境下(温度≥35℃,湿度≥80%)新兵100名为高温新兵组;高温高湿环境下服役2~3年的官兵100名为高温老兵组。受试者负重15kg,分次在3000m/15min,5000m/30min和10000m/45min内完成跑步训练。10000m训练后,根据受试者脑血流高低又分为脑血流增高组(24名)、脑血流降低组(46名)和脑血流正常组(230名)。训练前后行经颅多普勒血流诊断仪和脑电图仪检测。结果(1)3000m训练后3组受试官兵脑平均血流速度(Vm)较训练前均显著增高(P〈0.01),但5000m和10000m训练后,常温新兵组降低6%和16%,高温老兵组降低5%和15%,高温新兵组降低14%和31%,比常温新兵组和高温老兵组低(P〈0.01);(2)10000m训练前后3组脑电图均在正常范围,3组比较差异无统计学意义(P〉0.05);(3)10000m训练后脑血流增高组和降低组脑电图比正常血流组活跃(P〈0.01)。结论高温高湿环境下10000m训练对官兵的脑血流有较大的影响,但对脑电生理无影响,老兵在此环境下则表现出较好的热耐受力和适应性。Objective To observe the changes of cerebral blood flow (CBF) and electroencephalography of solders stationed in islands during training in hot and humid environment, so as to put forward theoretical basis for clinical prevention and treatment of heat illness. Methods One hundred recruit under normal ambient temperature ( Ta ) and relative humidity ( RH ) [ ( 27.4 ± 1.37 ) ℃, ( 50.25 ± 6. 76 )% ] , 100 recruits under high Ta and RH ( ≥ 35 ℃ , ≥ 80% ) , and another 100 officers and soldiers stationed in island for 2 to 3 years under high Ta and RH were studied. All the subjects had 3000 m/15 min ,5000 m/30 min ,and 10 000 m/45 min running each time with 15 kg of weigh loading. Before and after training, CBF and electroencephalography of the subjects were detected by transcrainial Dopple (TCD) and electroencephalogram (EEG). The data were analyzed among the above three groups, and according to different CBF after 10 000 m training increasing,reduceing and normal groups were derided. Results ( 1 ) The Vm of CBF were increased in all subjects after 3000m training (P 〈0.01 ) ;but were reduced 6% and 16% in normal Ta and RH recruits after 5000m and 10 000 m training,14% and 31% in high Ta and RH recruits for 2 to 3 years. The Vm in high Ta and RH recruits were lower than that in normal Ta and RH recruits and in high Ta and RH recruits for 2 to 3years (P 〈 0.01 ). (2) There were no significant differences among the three groups before and after the 10000m training in EEG and the wave shape and rhythm are regular. (3) After 10 000 m training, the EEG in increasing group and in reduceing group were activer than that in normal group ( P 〈 0.01 ). Conclusions High Ta and RH environment may had much effect on CBF that in and no effect on EEG after 10 000 m military training, while those who stationed for 2 to 3 years may have better heat tolerance and adaptability.
