中药芪丹通脉片对心肌缺血/再灌注损伤大鼠的影响

摘　　要：目的:探讨中药芪丹通脉片对大鼠缺血/再灌注(I/R)损伤心肌的影响。方法:采用结扎左冠状动脉前降支方法制备大鼠心肌I/R模型。将36只健康雄性SD大鼠随机均分为假手术组、模型组、恬尔心组以及芪丹通脉片高、中、低剂量组(浸膏干粉剂量分别为3.24、1.08和0.36 g.kg-1.d-1)。动态Ⅱ导联心电图监测I/R期间各组大鼠心率及ST段变化值。实验结束后采全血,采用酶联免疫吸附法(EL ISA)检测血清白细胞介素-10(IL-10)的含量,并用透射电镜观察心肌组织超微结构变化。结果:芪丹通脉片高、中、低剂量组可明显减慢大鼠I/R期间增快的心率,降低升高的ST段;减少炎性细胞浸润,减轻心肌超微结构损伤;并可增加血清IL-10含量,其中高剂量组作用最强(P<0.05或P<0.01),且芪丹通脉片高剂量组与恬尔心组的作用差异无统计学意义。结论:中药芪丹通脉片可改善受损心肌超微结构及异常心电图,促进内源性IL-10大量释放,抑制炎症反应,从而发挥心肌保护作用。Objective： To investigate the effects of Chinese herb Qidan Tongmai tablet（芪丹通脉片,QDTMT） on the plasma interleukin-10（IL-10） in the rat model of myocardial ischemia/reperfusion（I/R） injury.Methods： A model of myocardial I/R injury was reproduced by ligation of left anterior descending branch of the coronary artery.Thirty-six male SD rats were randomly divided into six groups（each n=6）： sham operation group,model group,diltiazem hydrochloride group,QDTMT high dose（3.24 g·kg^-1·d^-1） group,QDTMT medium dose（1.08 g·kg^-1·d^-1） group and QDTMT low dose（0.36 g·kg^-1·d^-1） group.The changes of heart rate and ST segmentⅡ in every group were observed with ECG.After the left anterior descending branch of coronary artery was occluded for 30 minutes followed by reperfusion for 2 hours,the whole blood was collected via right common carotid artery and the serum level of IL-10 measured by enzyme linked immunosorbent assay（ELISA）.At the same time,the myocardial ultrastructural alterations of rats in every group were observed with transmission electron microscope.Results： The faster heart rate and the higher ST segment during the period of I/R injury were decreased obviously and the contents of IL10 in serum were increased in the three QDTMT groups.Electron microscopic examination showed that pathologic changes of myocardiocytes in QDTMT groups were milder than those of the model group.The effects in QDTMT high dose group were best in these groups（P〈0.05 or P〈0.01）,and insignificant difference was shown between it and diltiazem hydrochloride group.Conclusion： Chinese herb QDTMT protects the injury of myocardial ischemia/reperfusion by maintaining morphologic integrity of myocardial cellular ultrastructure,improving the ECG,facilitating the expression of endogenous IL-10 and inhibiting the inflammatory reaction.
