摘  要:目的:观察高压氧预处理(HBOP)和低压缺氧预处理(HHP)对兔大脑中动脉闭塞(MCAO)后局灶性脑梗死程度及周围脑组织氧分压(PbtO2)的影响.方法:雄性家兔72只随机分为单纯MCAO组,HBOP+MCAO组(100%O2,2.5ATA,1h/d,5d)和HHP+MCAO组(4000m,61.6kPa,3d),每组24只,采用改良Bederson法建立家兔MCAO模型,分别于MCAO前、MCAO后1,3和10d观察脑MRI信号强度、脑梗塞灶体积及梗塞灶边缘PbtO2.结果:MRI结果显示MCAO后3,10d时HBOP+MCAO组和HHP+MCAO组缺血区异常信号范围较单纯MCAO组明显缩小,HBOP+MCAO组缺血区标准化信号强度(SIR)于3,10d时分别为1.62±0.09,1.17±0.10;HHP+MCAO组缺血区SIR于3,10d时分别为1.64±0.06,1.20±0.09;均低于单纯MCAO组的1.83±0.07,1.36±0.06(P<0.05).TTC染色显示的梗塞灶体积在3和10d时HBOP+MCAO组和HHP+MCAO组小于单纯MCAO组;HBOP+MCAO组家兔梗塞灶边缘PbtO2于1,3和10d时分别为(8.27±0.58),(11.92±1.27)和(16.12±1.42)mmHg;HHP+MCAO组家兔梗塞灶边缘Pb-tO2分别为(8.46±0.90),(10.77±1.19)和(15.33±1.29)mmHg,均高于单纯MCAO组的(7.03±1.06),(8.43±0.86)和(11.83±1.28)mmHg(P<0.05).结论:HBOP及HHP对家兔MCAO后局灶性脑缺血具有一定的防治作用.AIM: To observe the extent of infarct and the variations of partial pressure of brain tissue oxygen ( PbtO2 ) around the infracted zone after permanent middle cerebral artery occlusion (MCAO) in rabbits with hyperbaric oxygen preconditioning ( HBOP ) and hypobaric hypoxia preconditioning ( HHP ). METHODS: Seventy-two male rabbits were randomly divided into 3 groups: simple MCAO group ( n = 24 ), HBOP + MCAO group (100% 02, 2.5 ATA, 1 h/d, 5 d, n=24) and HHP+ MCAO group (4000 m, 61.6 kPa, 3 d, n=24). The signal intensity of MRI, infarct size and PbtO2 were detected at 1, 3 and 10 d after permanent MCAO. RESULTS: Compared with the simple MCAO group, the abnormal signal zone of Ta WI in MRI which indicated ischemia and the TIC staining which indicated infarcted size reduced significantly at 3 and 10 d after permanent MCAO. The Signal intensity ratio (SIR) in the simple MCAO group were (1.83±0.07), (1.36±0.06) at 3 and 10 d after permanent MCAO, which were significantly decreased after HBOP [(1.62±0.09), (1.17+0.10)] and HHP [(1.64±0.06), (1.20 ± 0.09 ) ] ( P 〈 0.05 ). PbtO2 around the infraeted regions in the simple MCAO group were (7.03 ± 1.06), (8.43 ±0.86), and ( 11.83 ± 1.28) mmHg at 1, 3 and 10d after permanent MCAO, which were significantly increased after HBOP and HHP (P 〈 0. 05 ). After permanent MCAO, the PbtO2 were (8.27±0.58), (11.92±1.27), and (16.12±1.42) mmHg in HBOP group, and (8.46 ±0.90), (10.77±1. 19), and ( 15.33±1.29) mmHg in HHP group. CONCLUSION: HBOP and HHP have an preventional and therapic effect on focal cerebral ischemia after permanent MCAO in rabbits.
