氢氧化钙治疗年轻恒牙意外露髓的临床观察

摘　　要：目的：研究Dycal治疗年轻恒牙意外露髓的临床效果。方法：将60颗髓室意外露髓患牙随机分为Dycal组38颗和对照组22颗,露髓处分别采用Dycal和氧化锌丁香油糊剂垫底,然后均用氧化锌丁香油酚水门汀暂封。观察一周后,治疗牙均无自发痛、夜间痛。然后去除部分氧化锌丁香油酚水门汀,剩下一层作第二层垫底材料,并作永久性（FujiⅨ）充填,所有治疗牙均随访观察1.5-2年。结果：Dycal组成功25颗,进步11颗,失败2颗,其疗效明显优于对照组（P〈0.05）;医源性和龋源性意外露髓之间,其疗效差异无显著性（P〉0.05）;露髓直径小于0.2mm者及0.2-0.5mm者,其疗效均明显优于0.5mm以上者（P〈0.05）。结论：Dycal是治疗年轻恒牙意外露髓较好的一种材料,其疗效与露髓大小有关。Objective：Research on the clinical effect of immature permanent teeth with accidentally exposed dental pulp treated by Dycal.Method： Randomly divide 60 teeth with exposed pulp into two groups.One is Dycal group of 38 teeth and the other is comparative group 22 teeth.Separately put Dycal and Znic Oxide Eugenol Paste on the bottom of exposed pulp.And then temporarily seal by Znic Oxide Eugenol Cement（ZOE）.After one-week observation,the treated teeth are all without spontaneous pain and night pain.Remove some of the Znic Oxide Eugenol Cement,leaving first layer as the bottom material of the second layer,and filling up permanently（Fuji Ⅸ）.All the treated teeth are follow-up observed for 1.5 to 2 years.Result： For Dycal group,25 teeth succeeded,11 progressed and 2 failed. The curative effect is obviously better than comparative group（P〈0.05）;the difference of curative effect between clinical origin and caries origin is not obvious（P〉0.05）;for the exposed pulp teeth whose diameter is less than 0.2mm and between 0.2 and 0.5mm,the curative effect is remarkably better than the ones whose diameter is more than 0.5mm（P〈0.05）.Conclusion： Dycal is a comparatively good material on immature permanent teeth with accidentally exposed pulp.The curative effect is depends on the size of the exposed pulp.
