摘 要:目的阐明转铁蛋白受体(CD71,TfR)在过敏性紫癜性肾炎(NHSP)肾组织中表达的分子构象特征及其与IgA分子构象特征之间的相互关系.探讨其在NHSP免疫发病机制中的作用。方法122例肾活检患者根据临床表现和肾活检病理诊断分为NHSP组40例、IgA肾病组44例、无IgA沉积肾病组(对照组)38例。运用免疫荧光双套色法标记的抗体在激光共聚焦荧光显微镜下观察四甲基罗丹明(TRITC)标记CD71和异硫氰荧光素(FITC)标记IgA在肾组织上的分子表达。结果CD71在对照组患者的肾小球上呈现微弱表达或不表达,在NHSP组和IgA肾病组呈现阳性高表达,且激光共聚焦荧光显微镜下观察到CD71的表达与IgA表达呈现共位。结论CD71是IgA的受体之一,其在NHSP患者肾小球上呈现高表达,且其分子构像与IgA分子呈现共位状态,提示CD71参与了NHSP的免疫发病过程。Objective To study the molecular pathological features of CD71 and the correlation between IgA deposition and CD71 expression in nephritis secondary to Henoh-Scholein purpura (NHSP), and to investigate the role of CD71 in the pathogenesis of NHSP in order to guide the treatment strategies. Methods According to the clinical manifestations and pathological diagnosis, 122 renal biopsy specimen could be categorized to NHSP (40 cases), IgA nephropathy (IgAN, 44 cases) and no IgA deposited nephritis (the control group, 38 cases). Double-staining immunofluorescence using TRITC-conjugated anti-CD71 antibody and FITC-conjugated anti-human IgA antibody were performed to illustrate the expression of CDT1 and IgA respectively by eonfoeal microscopy. Results CD71 expression had not been found in the control group. Its ex- pression was positive in both the NHSP and IgAN group by the confocal miscroscopy and it had been found that IgA and CD71 were co-expressed. Conclusion CD71, an IgA receptor, is highly expressed by the glomeruli of NHSP and co-expressed with IgA. It suggests that CDT1 may play an important role in the immunopathogenesis of NHSP.
