摘 要:目的探讨人肝癌组织胰岛素样生长因子Ⅱ(IGF-Ⅱ)基因P3,P4启动子表达及其与p53基因突变的关系。方法应用半定量RT-PCR、PCR、直接测序分别检测7例正常成人肝组织、23例乙型肝炎病毒阳性肝癌组织IGF-Ⅱ基因P3,P4启动子特异的转录子表达及p53基因突变状况。结果①绝大多数肝癌组织P3和P4mRNA表达量显著上调,平均值均明显高于正常成人肝组织(P<0.01);②肝癌组织p53基因的突变率为30.4%(7/23)。③23例肝癌组织中,具有p53基因突变的标本P3,P4mRNA表达水平分别明显高于无p53基因突变的肝癌组织(P<0.05)。结论①IGF-Ⅱ基因P3和P4再激活是大多数乙型肝炎病毒阳性肝癌的重要特征;②肝癌组织P3,P4mRNA显著上调与p53基因突变密切相关,后者可能是P3,P4再激活的转录调控机制之一。Objective To investigate the expression of P3 and P4 promoter of insulin - like growth factor - Ⅱ( IGF - Ⅱ ) gene and its correlation with p53 gene mutation in human hepatocellular carcinoma. Methods Tumorous tissues from 23 consecutive patients with hepatocellular carcinoma and hepatitis B virus infection, and 7 normal liver tissues were determined by pathological examinations. IGF - Ⅱ transcripts from the P3 and P4 promoters and mutations of p53 gene in all hepatic samples were evaluated using a semiquantitative reverse - transcription polymerase chain reaction assay, PCR and direct sequencing, respectively. Results ①P3 and P4 mRNA expression significantly increased in most of hepatoceUular carcinoma samples as compared with normal samples. ②Mutations of p53 gene were detected in 30. 4% (7/ 23 )of HCC tissues. ③In 23 hepatocellular carcinoma tissues, P3 and P4 mRNA levels in HCC tissues with mutations of p53 gene were significantly higher than those in HCC tissues without mutations of p53 gene( P 〈 0. 05). Conclusion.①The reactivation of P3 and P4 promoters of IGF - Ⅱ is an important characteristics in most human hepatocellular carcinoma with hepatitis B virus infection. ②Increased expression of P3 and P4 is associated with mutations of p53 gene that may contribute to the expression activation of P3 and P4 transcript in HCC tissues.
