CAS-R-2无框架立体定向血肿碎吸术治疗脑内小血肿疗效分析

摘　　要：目的探讨计算机辅助立体定向血肿碎吸引流术治疗高血压脑出血幕上小血肿的临床价值。方法应用CAS-R-2无框架立体定向血肿碎吸引流术治疗高血压脑出血幕上小血肿48例（微创手术组）。与同期非手术治疗的在出血部位、出血量、入院时意识状况相匹配的45例病人（非手术组）进行回顾性对比研究。结果生存病例出院时用Barthel日常生活能力指数评估ADL（active daily liver）能力。得分61～100分（生活自理）病例数,微创手术组为37例,占本组总病例数的77.08%,非手术组为22例,占本组总病例数的48.89%。两组比较差异有统计学意义（χ^2=6.791,P〈0.05）。结论对于高血压脑出血幕上小血肿,CAS-R-2无框架立体定向血肿碎吸引流术是一种安全、简便、有效的微创手术。Objective To evaluate the clinical application of computer assisted frameless stereotactic aspiration and drainage for small supratentorial hematomas resulting from hypertensive intracerebral hemorrhage.Methods Forty-eight patients of small suprateontorial hematoma were treated by Model CAS-R-2 frameless stereodirected instrument plus stereotactic aspiration and hematoma cavity drainage（as trial group）.Another simultaneous 45 patients was treated with non-surgery as control group.Results In the trial group,77.08% of the cases reached a score of 60-100 of ability of daily life（ADL）（i.e.self-care in daily life）,but the counterpart was only 48.89% in the control group.There was a statistically significant difference in ADL between the two groups（χ^2=6.791,P〈0.05）.Conclusion Model CAS-R-2 frameless stereodirected instrument aided stereotactic aspiration and hemotoma cavity drainage is a minimal invasive procedure with advantage of simplicity,safety and effectiveness for the treatment of small supratentorial hemotomas.
