宫颈电热圈环切在宫颈癌前病变诊治中的临床应用

摘　　要：目的探讨宫颈电热圈环切(loopelectrosurgical excision procedure,LEEP)诊治宫颈癌前病变(cervicalintraepithelial neoplasia,CIN)的临床价值。方法对阴道镜下活检诊断为CIN及TCT异常、而阴道镜检查不满意(指宫颈鳞柱交界不可见)的患者75例采用LEEP术进行诊治;观察手术时间、出血量、术后疗效及诊断符合率。结果LEEP术治疗CIN一次成功率高,时间短,出血少,无术后并发症发生,阴道镜下活检与LEEP标本的病理诊断有差异。结论LEEP诊治CIN操作简单、安全,术后并发症少,效果佳,可提供完整的病理标本,尤其对早期诊断宫颈原位癌或微小浸润癌具有极高的临床价值。Objective：To investigate the clinical value of loopelectrosurgical excision procedure（LEEP） in the diagnosis and treatment of cervical intraepithelial neoplasia（CIN）.Methods：We apply LEEP to deal with 75 casess whose biopsy CIN and thin-prep cytoloy test（TCT） are abnormal but whose vaginoscopy are disaffect（the borderline between scalelike and stylolitic is unclear）.We observe the operation time,the bleeding amount,the postoperative effect and the diagnose accordance rate.Results：When we using LEEP to treat CIN the once achievement ratio is high,the time is short,the bleeding is little,the postoperative complications do not happen.The pathologic diagnosis between colposcope biopsy sample and LEEP sample are difference.Conclusions：When we using LEEP to treat CIN the performance is simple and safe,the postoperative complications is few,the effect is fine.LEEP can provide intact pathology sample,especially benefit the early diagnosis to carcinoma in situ of cervix and microinvasion.
