摘 要:目的评价血小板源性生长因子(PDGF)抗体和血管内皮生长因子(VEGF)抗体对实验性增生性玻璃体视网膜病变(PVR)的预防作用。方法将兔制成PVR模型,将一定浓度的PDGF抗体、VEGF抗体及BSS液分别在当时及第7d注入兔眼的玻璃体腔,每日观察兔眼情况,处死兔子做眼病理切片及免疫组织化学染色。结果间接检眼镜观察可见对照组与实验组均出现视网膜脱离。玻璃体腔内注药后第21d、28dPVR分级实验组与对照组差异有统计学意义(P<0.05),在制作动物模型时直接注入和第7d时注入差异无统计学意义(P>0.05)。病理检查所见与间接检眼镜所见相符。免疫组织化学染色计算阳性细胞百分率,亦得出相同结果。结论玻璃体腔内注入PDGF抗体、VEGF抗体均可预防PVR的进展,注药时间对于PVR的发展无明显影响。Objective Proliferative vitreoretinopathy is the most common complication of rhegmatogenous retinal detachment(RRD) ,and the occurrence and development of PVR are associated with several factors. This study was done to evaluate the preventive effect of platelet-derived growth factor (PDGF) antibody and vascular endothelial growth factor (VEGF) antibody on experimental proliferative vitreoretinopathy (PVR). Methods Firty-two rabbits were used in this experiment. Vitrectomy were performed in the left eyes of 24 rabbits to create the retinal detachemnt model. 0. 1 mL PDGF antibody was injected intravitreally at the 1st day and the 7th day following modeling in group D,E respectively, and 0. 1 mL VEGF antibody was injected at a same way in group F, G respectively. PDGF or VEGF antibody was injected intravitreally in 6 normal eyes in group A or group B respectively,and 0. 1 mL BSS was injected in 6 eyes of rabbits in group C. The clinical examination of animal fudus was taken with ophthalmoscope everyday and the PVR was graded according to the Fastenberg criteria. The rabbits were sacrificed at the 28th day and the vitreous specimen was pathologically examinated under the light microscope. Immunohistochemistry was performed to assess the numbers of RPE and glial cells. Results The grade V of PVR was found in rabbit eyes of group C (33%) ,and over grade Ⅲ of PVR was seen in experimental eyes,showing a severer PVR in various experimental groups compared with control group at the 21st day and the 28th day (P 〈0.05) ,but no significant difference was seen in PVR in different time of drug injection(P 〉0.05). Immunohistochemical result showed that the rates of CK positive cells and GFAP positive cells in group D, E, F,G were evident lower than group C,indicating less RPE and glial cells in group D,E, F,G. Conclusion PDGF antibody and VEGF antibody inhibit the experimental PVR. This study provides an useful guideline for prevention of PVR.
