彩色血管能量成像检测甲状腺癌血流及其与凋亡抑制因子的关系

摘　　要：目的应用彩色血管能量成像(CPA)技术检测甲状腺癌内的血流及其与凋亡抑制因子(survivin)的相关性。方法应用CPA技术检测50例甲状腺癌患者及30例甲状腺腺瘤患者的血流信号,及其阻力指数(RI),采用免疫组化法检测该组织中survivin的表达。结果甲状腺癌survivin的阳性表达率(68%)显著高于甲状腺腺瘤组(33.3%,P<0.05),甲状腺癌中survivin的阳性表达率与病理的高分期和淋巴结转移密切相关(P均<0.05)。甲状腺癌中的survivin表达强度与CPA血流分级及RI值均呈正相关(r1=0.827,r2=0.809,P<0.05)。结论应用CPA技术与免疫组化技术二者的有机结合可对甲状腺癌进行术前、术后多方面评估,从而为评价其临床应用提供参考。Objective To evaluate the relationship between survivin （an inhibitor of apoptosis protein） expression in thyroid carcinoma and color power Doppler angioimaging （CPA） of blood flow. Methods Blood signals and RI in 50 thyroid carcinoma patients and 30 cases with adenoma were preoperatively measured on CPA whereas the survivin was stained with immunohistochemistry after operations. Results The positive expression of survivin intensity （68M） in thyroid carcinoma group was significant higher than that in the adenoma group （33.3%, P〈 0.05 ）, and this positive intensity in thyroid carcinoma group associated closely with the high TNM classifications and the lymph node metastasis （all P〈0.05） . Meanwhile, the expression of survivin intensity in thyroid carcinoma group correlated positively to both the CPA degrees and their RI values （r2 = 0.827, r2 = 0.809, P 〈 0.05） . Conclusions The efficient integration of CPA and immunohistochemistry can help us to assess the patients＇ conditions before and after their operations, which may provide some information important in clinical.
